彩色多普勒血流显像对椎动脉起始段狭窄支架置入术后的疗效评价
Evaluation of Therapeutic Effect of Stent Implantation in the Initial Segment of the Vertebral Artery by Color Doppler Flow Imaging
【摘要】 目的应用彩色多普勒血流显像(CDFI)评价椎动脉起始段狭窄支架置入术后的疗效。方法对102例椎动脉起始段狭窄的患者行经皮血管内球囊成形支架置入术,分别于术前、术后6个月行CDFI检查,观察术前及术后6个月椎动脉起始段、椎间段的收缩期峰值流速(PSV)、舒张末期流速(EDV)、血流阻力指数(RI)及起始段管径变化。统计术后6个月狭窄再发生率。评价椎动脉起始段狭窄支架置入术后的疗效。结果椎动脉起始段PSV和EDV由术前的(302.2±104.6)cm/s和(82.3±43.2)cm/s降为(98.0±34.3)cm/s和(24.5±12.4)cm/s(P<0.01);椎动脉椎间段PSV和EDV由术前的(40.2±15.2)cm/s和(17.6±8.5)cm/s升为(56.5±18.2)cm/s和(20.3±9.7)cm/s(P<0.01);椎间段RI由(0.56±0.10)升为(0.64±0.10)(P<0.01);起始段管腔内径由(1.10±0.35)mm增为(2.80±0.40)mm(P<0.01)。6个月后起始段狭窄再发生9例,发生率8.8%。结论 CDFI可以有效地评价椎动脉支架治疗术后疗效,及时发现再狭窄的发生。
【Abstract】 Objective To evaluate the therapeutic effects of stent implantation in stenosis of the initial segment of the vertebral artery by using color doppler flow imaging(CDFI).Methods A total of 102patients with stenosis of the initial segment of the vertebral artery underwent percutaneous transluminal angioplasty(PTA).CDFI was performed in these patients before and6months after the operation.The data was collected and analyzed,including the peak systolic velocity(PSV),end-diastolic velocity(EDV),blood flow resistance index(RI)and changes in the diameter of the initial segment and the intervertebral segment of the vertebral artery.The rate of stenosis recurrence was obtained 6months after operation.Results Six months after operation,PSV and EDV of the initial segment of the vertebral artery were decreased from(302.2±104.6)cm/s and(82.3±43.2)cm/s before the operation to(98.0±34.3)cm/s and(24.5±12.4)cm/s(P<0.01);PSV and EDV of the intervertebral segment of the vertebral artery were increased from(40.2±15.2)cm/s and(17.6±8.5)cm/s before the operation to(56.5±18.2)cm/s and(20.3±9.7)cm/s(P<0.01);RI of the intervertebral segment of the vertebral artery was increased from(0.56±0.10)to(0.64±0.10)(P<0.01);the diameter of the initial segment of the vertebral artery was increased from(1.10±0.35)mm before the operation to(2.80±0.40)mm(P<0.01).Six months after the operation,stenosis re-occurred in 9out of the 102cases and the rate of recurrence was 8.8%.Conclusion CDFI can be used to evaluate the therapeutic effects of stent implantation in the vertebral artery and detect the recurrence of stenosis in time.
