亚低温对窒息大鼠心肺复苏后S100B蛋白及胶质纤维酸性蛋白表达的影响

【摘要】 目的:探讨亚低温对窒息大鼠心肺复苏后血清、大脑皮层S100B蛋白及胶质纤维酸性蛋白(GFAP)表达的影响。方法:60只SD大鼠随机分为假手术组(A组)、常温组(B组,常规复苏)、保存性低温组(C组,常规复苏,心跳骤停时给予亚低温治疗)、复苏性低温组[D组,常规复苏,自主循环恢复(ROSC)后30min给予亚低温治疗],各组于心跳骤停时和ROSC后2、4h取血,处死大鼠取脑组织,观察各时点血清大脑皮层S100B蛋白、GFAP的动态变化。结果:(1)B、C、D各组血清S100B蛋白从ROSC后2h起开始升高,并持续上升至4h,与A组比较,各时点血清S100B蛋白含量及大脑皮层S100B蛋白表达均显著增高(P<0.05)。(2)与B组比较,C组ROSC后2h血清S100B蛋白含量降低,差异无统计学意义(P>0.05),ROSC后4h血清S100B蛋白含量显著降低(P<0.05),皮层S100B蛋白表达显著减少(P<0.05)。(3)与A组比较,B、C、D各组ROSC后4h大脑皮层GFAP表达均显著增加(P<0.05)。结论:大鼠血清、大脑皮层组织S100B蛋白和GFAP在ROSC后早期即有表达,可以作为诊断脑损伤及判断预后的敏感标志物;亚低温可减轻窒息大鼠心肺复苏后脑损伤。

【Abstract】 Objective To explore the effects of mild hypothermia on expression of S100B and GFAP in serum and brain after cardiac arrest (CA)-cardiopulmonary resuscitation (CPR) in anoxia-induced rats.Methods Sixty adult SD rats were randomly divided into 4 groups: sham-operation group (group A), normothermia group (group B, CPR), instant hypothermia group (group C, used hypothermia on CA immediately), and resuscitation hypothermia group [group D, used hypothermia after 30 min of restoration of spontaneous circulation (ROSC)]. Blood was taken at CA times, 2 h and 4 h after ROSC, respectively. S100B and GFAP expression were observed at each time point. Results The serum levels of S100B protein was increased significantly at 2 h after ROSC and continue increased at 4 h in group B, C and D. The expression of S100B protein in serum and cerebral cortex were increased significantly at each time point when compared with group A (P < 0.05). The serum levels of S100B protein decreased at 2 h after ROSC in group C when compared with group B, but there was no significant difference between them (P > 0.05). The expression of S100B protein in serum and cerebral cortex was decreased significantly at 4 h after ROSC when compared with group B (P < 0.05). Compared to group B, the expression of GFAP protein in cerebral cortex was increased at 4 h after ROSC in group B, C and D (P < 0.05). Conclusions The S100B and GFAP in serum and cerebral cortex were expressed at the early phase after ROSC in rats, which can be used as a sensitive marker for early diagnosis of cerebral injury and prognosis. Mild hypothermia could alleviate the cerebral injury in anoxia-induced rats after cardiopulmonary resuscitation.
