【摘要】 目的:探讨外周血CD64和C-反应蛋白(CRP)的表达对烧伤感染患者的临床诊断价值。方法:选择97例烧伤患者,分为感染组48例(其中脓毒症组26例,局部感染组22例)和对照组49例。各组患者在入院治疗前后取外周静脉血1.5mL,分别检测中性粒细胞CD64的表达水平及CRP。Logistic回归筛选相关指标,并建立相应回归方程,R0C曲线分析临床特性。结果:感染组CD64、CRP的表达水平显著高于对照组(P﹤0.001);烧伤感染患者CD64、CRP的表达与感染的严重程度相关,感染的程度越重,两者的表达水平越高,即脓毒症组CD64、CRP的表达水平较感染组高;治疗后CRP和CD64明显较治疗前下降。建立回归方程Y=-6.451+3.825X1+4.222X2,新变量Y诊断烧伤感染的敏感性和特异性分别为95.80%和89.80%。结论:烧伤感染患者外周血CD64、CRP的表达水平明显升高,监测外周血CD64、CRP有助于烧伤感染的临床诊断,运用Logistic回归和R0C曲线综合分析可提高诊断的敏感性和特异性。更多还原

【Abstract】 Objective To explore the clinical diagnostic value of neutrophil CD64 and C-reactive protein(CRP) in peripheral blood for the infection in patients with burns.Methods 97 patients with burns were divided into control group(n = 49) and infection group(n = 48) which were made up of pyaemia group(n = 26) and local infection group(n = 22).The peripheral blood samples of 1.5 mL were taken from all the patients before and after treatment,and levels of CD64 and CRP were measured.Results The expressions of CD64 and CRP in infection group were significantly higher than those in control group(P < 0.001),and were related to the severity of the infection,which meant that they were higher in pyaemia group than in local infection group.After the treatment,the expressions of CD64 and CRP were decreased obviously.According to the regression equation Y =-6.451 + 3.825X1 + 4.222X2,the sensitivity and specificity of new variable Y were 95.80% and 89.80%.Conclusion The expressions of neutrophil CD64 and CRP in peripheral blood were increased significantly in patient with burns complicated with infection,which were contribute to the diagnosis of it.[更多还原
