【摘要】 目的探讨进展期胃癌的CT表现及手术可切除性的术前评估价值。方法分析100例进展期胃癌的CT表现,对术前评估与手术结果进行对比研究。结果胃底贲门癌26例,胃体癌39例,胃窦癌20例,病变占据两个分区以上者15例。100例胃癌均显示胃壁有不同程度的增厚,部分胃壁有软组织肿块形成,黏膜面有溃疡形成,胃腔及贲门狭窄,贲门管壁增厚,食管下段受累以及周围组织器官侵犯等。CT检查对进展期胃癌的诊断符合率高,肿瘤检出率可达100%,术前判断为可切除组的病例中手术切除率达91.2%,手术前判断为不可切除组的病例中手术切除率仅为15.0%。结论CT检查对肿瘤可切除性的术前评估有较高的临床价值,值得推广应用。更多还原

【Abstract】 Objective To discuss the CT manifestation and the preoperative assessment of resectability of advanced gastric carcinoma.Methods The CT manifestation of 100 cases with advanced gastric carcinoma were analyzed,compared preoperative evaluation and surgical outcomes.Results Twenty-six cases with gastric cardia carcinoma,thirty-nine cases with gastric body carcinoma,twenty cases with gastric antrum carcinoma,it only had 15 cases that the lesions took up more than two partitions;One hundred cases of gastric cancer showed that the gastric wall had varying degrees of thickened,some gastric wall had formed of soft tissue mass,mucosa surface had formed of ulcer,gastral cavity and cardia stenosis,cardia walls thickened,oesophagus lower segment involvement and the surrounding tissue invasion,et al.CT had the high accurate rate in diagnoses of advanced gastric carcinoma,and the detectable rate of tumour by CT was 100%,the excision rate in the predicted operable group was 91.2%,the non-excision rate was 15.0% in predicted unresectable group.Conclusion Preoperative CT diagnosis of gastric carcinoma has important clinical significance.CT provides a high reference value for assessing the resectability of gastric carcinoma,and it is worthy of widespread used.更多还原
