【摘要】 目的 探讨原发性肝血管肉瘤(PHA)的CT表现。方法 回顾性分析6例PHA的CT表现,并结合文献复习及病理分析。结果 肿块型5例,伴子灶2例,共7处病灶,弥漫多结节型1例。CT均表现为类圆形低密度区,均匀或不均匀,肿块型中3例边界尚清、2例模糊,结节型多数结节边界尚清。3例肿块型病灶动脉期呈中心区小片状或结节不均匀轻度强化,1例伴边缘轻度强化,门脉期持续强化填充伴2例边缘区强化扩充,延迟期持续填充而呈欠均匀略低密度1例、略高密度灶2例;2例肿块型病灶呈中心区小片状或结节不均匀明显强化,门脉期持续强化填充伴边缘区强化扩充,延迟期持续填充而呈欠均匀略高密度灶;弥漫多结节型病灶动脉期大部分呈中心小斑片状或结节明显强化,少数病灶伴或仅呈边缘结节状明显强化,门脉期及延迟期持续强化填充呈均匀或欠均匀等、略高密度灶。5例均未见明显累及门脉、胆管征象。结论 PHA的CT表现具有一定特征性,结合临床病史及体征,应考虑到PH A的可能性,最终确诊依赖于病理。更多还原

【Abstract】 Objective To explore the CT performances of primary hepatic angiosarcoma(PHA).Methods CT features in 6 patients with PH A confirmed by pathological results were retrospectively analyzed,and some related literatures were also reviewed.Results Five masses with 2 nodules of PHA in 5 patients were showed on CT images.The diffused multiple nodules were showed in remaining one patient.On plain CT images,all lesions appeared as heterogeneous or homogeneous hypodensity,with clear boundary in 3 masses and in most diffused nodules and without that in 2 masses.In arterial phase,patchy or nodular mild(n=3) or obvious(n=2) enhancement at the central area with heterogeneity was found,and mild border enhancement was seen in one.In portal vessel phase,the sustained enhancement was showed in 5 masses with expanded border enhancement in 4.In delayed phase,the sustained enhancement was also found with slight hypodensity and heterogeneity in one and with slight hyperdensity in 4.In the patient with diffused multiple nodules,in arterial phase most nodules appeared as patchy or nodular obvious enhancement at the central area,however few lesions exhibited nodular obvious enhancement in peripheral portion;in portal vessel and delayed phases,the sustained enhancement was seen with heterogeneous or homogeneous iso-or hyperdensity.Conclusion PHA have some characteristic CT features,however the diagnosis depends on CT and clinical characters,even on the pathology.更多还原
