静脉输液港使用并发症的X线造影表现及问题处理

【摘要】 目的 通过X线造影发现并明确静脉输液港使用中相关并发症的原因,分析其对应的临床问题并指导处理方法。方法 收集静脉输液港使用过程中出现异常病例65例,经外接管注入欧乃派克约20mL并同时连续采集图片,分析X线造影表现。结果 本组病例中38例X线造影表现异常。纤维蛋白鞘形成27例,其中导管头端局部纤维蛋白鞘形成覆盖侧孔21例,血管内导管弥漫纤维蛋白鞘形成6例;导管打折4例;导管断裂2例;静脉港注射座周围对比剂外溢1例;导管内血栓形成1例;管头位置异常3例,包括大血管变异1例,管头进入乳内静脉2例。结论 静脉输液港临床应用越来越广泛,X线造影检查是发现和明确使用中并发症的原因并指导正确处理的可靠方法。

【Abstract】 Objective To clarify the reason of venous infuse port related complications,to analyse the corresponding clinical problems and to guide the processing method by X-ray angiography.Methods 65 patients with venous infuse port related complications were enrolled in this study.Omnipaque(about 20 mL) was injected into the port through a external linked pipe and continuous images were acquired at the same time.Then the angiography findings were analysed.Results 38 cases showed abnormal performances on X-ray angiography.Fibrin sheath formation in 27 cases,Among the 27 patients,the local fibrin sheath of the catheter tip to form cover the side holes in 21 cases,to fill with the intravascular catheter fibrin sheath formation in six cases.Catheter discount in 4 cases;catheter fracture in two cases;spill over around the intravenous injection seat of contrast in 1 case;thrombosis formation in catheter in 1 case;abnormal position of the tube head in 3 cases,including the great vessels variation in 1 case,the tube head into the breast vein in 2 cases.Conclusion X-ray angiography examination is an important method to find and clarify the reason of venous infuse port related complications.
