【摘要】 目的 分析乳腺病变DCE-MRI与X线摄影的影像学特征,评价两者对乳腺疾病的诊断价值。方法 回顾性分析32例41处乳腺病变患者的DCE-MRI及X线摄影表现,DCE-MRI包括剪影技术及高分辨成像扫描,乳腺X线采取常规摄影。结果 38处病灶DCE-MRI正确诊断,与病理结果相符,准确率为93%;28处病灶X线摄影正确诊断,与病理结果相符,准确率为68%,两者差异有统计学意义,χ~2=7.77,P<0.05。在乳腺癌的诊断方面,DCE-MRI的敏感性高于X线摄影,分别为94%与75%;但特异性低于X线摄影,分别为92%与96%。结论 乳腺病变DCE-MRI的准确性高于乳腺X线摄影,对乳腺癌的诊断其敏感性较高而特异性较低,两者结合有助于提高术前的正确诊断。更多还原

【Abstract】 Objective To evaluate the diagnostic value of dynamic contrast-enhancement magnetic resonance imaging(DCE-MRI)and mammography in breast lesions by analyzing their imaging characteristics.Methods Mammography and DCE-MRI of 32 patients with 41 breast lesions were retrospectively analyzed.DCE-MRI included the digital subtraction MRI and high spatial resolution magnetic resonance imaging.Mammography was performed with conventional method.Results With DCE-MRI,38 lesions were correctly diagnosed comparing with pathological results,the diagnostic accuracy was 93%(38/41).With mammography,28 lesions were correctly diagnosed,and the diagnostic accuracy was 68%(28/41).Diagnostic accuracy of the two methods was significantly different(χ~2=7.77,P<0.05).In the diagnosis of breast cancer,the sensitivity of DCE-MRI(94%) was higher than mammography(75 %),and the specificity of DCE-MRI(92%) was slightly lower than mammography(96%).Conclusion The diagnostic accuracy of DCE-MRI in breast lesions is higher than mammography.In detecting breast cancer,DCE-MRI has a higher sensitivity and slightly lower specificity comparing with mammography.The combination of the two methods is helpful to improve diagnostic accuracy of breast lesions.更多还原
