【摘要】 目的 回顾性分析胃印戒细胞癌多排螺旋CT特点及TNM分期的能力。方法 收集本院病理证实的40例胃印戒细胞癌的CT资料,分别从胃印戒细胞癌的发病部位、病灶大小、形态、强化模式以及T、N分期总结其特点,并与病理对照。结果 胃印戒细胞癌病灶CT平扫密度大于正常胃壁(P<0.05),在对均匀强化病灶、不均匀强化病灶高强化区、低强化区及正常胃壁的强化模式分析中,发现各组差别在三期扫描中具有统计学意义上的差异(P<0.05),不均匀强化病灶低强化区具有延迟强化的特点。CT准确评价了18例患者的肿瘤浸润深度,对T4期的诊断准确性达到93.9%。结论 多排螺旋CT对胃印戒细胞癌的诊断和临床分期具有一定的帮助,胃印戒细胞癌病灶强化模式多样化,不均匀强化病灶低强化具有延迟强化的特点,在肿瘤的T4与非T4分期上有比较乐观的结果。更多还原

【Abstract】 Objective To retrospective analysis the features and TNM staging ability of gastric signet ring cell carcinoma on multi-detector spiral CT.Methods CT data of 40 cases of gastric signet ring cell carcinoma proven pathologically were collected,inorder to analyze the characteristics of gastric signet ring cell carcinoma on the lesion location,shape,contrast-enhancement patterns,T and N staging,respectively,and compare with pathology.Results The plain CT scanning density of gastric signet ring cell carcinoma lesions was greater than that of the normal gastric wall(P<0.05).By the analysis of enhancement patterns of the homogeneous enhance lesions,heterogeneous enhance lesions,low enhancement lesion and normal gastric wall,statistically significant differences(P<0.05) were found in the three phase scans.Low enhancement areas of the heterogeneous enhancement lesions showed the characteristics of the delayed enhancement.The tumor infiltration depth was accurately evaluated in 18 patients,and diagnosis accuracy of T4 stage was up to 93.9%.Conclusion Multi-detector spiral CT may assist to diagnosis and clinical staging of gastric signet ring cell carcinoma.The enhancement patterns of gastric signet ring cell cancer are diversified,and low enhancement areas of the heterogeneous enhancement lesions have the characteristics of delayed enhancement.There is optimistic result in the distinguishing of T4 from non-T4 staging of gastric signet ring cell cancer.更多还原
