膀胱癌表观扩散系数值与PCNA、Ki-67表达的相关性研究
Correlation of ADC value with the expressions of PCNA and Ki-67 in bladder cancer
【摘要】 目的 通过测定膀胱癌肿瘤组织中增殖细胞核抗原(PCNA)及Ki-67的不同表达程度及与其表观扩数系数(ADC)值相关性,探讨应用ADC值从分子水平诊断膀胱癌恶性程度的价值。方法 对45例膀胱癌患者术前行常规MRI检查并进行ADC值的测定(b=800 s/mm2)。采用免疫组织化学染色方法(SP法)检测45例膀胱癌标本中PCNA、Ki-67的表达情况。结果 膀胱癌肿瘤组织PCNA、Ki-67表达级别增高,ADC值降低,PCNA、Ki-67高表达组ADC值分别为(0.79±0.12)×10-3 mm2/s、(0.77±0.10)×10-3mm2/s,低表达组ADC值分别为(1.12±0.15)×10-3mm2/s、(1.05±0.08)×10-3 mm2/s,PCNA、Ki-67表达程度与肿瘤的ADC值呈显著负相关(r=-0.832、r=-0.741,P<0.05)。结论 膀胱癌ADC值能从分子水平反映细胞增殖程度,为膀胱癌术前恶性程度及预后评估提供参考。
【Abstract】 Objective To study the value of apparent diffusion coeffcient(ADC) and the correlation between ADC value and expressions of PCNA and Ki-67 in bladder cancer.Methods We included 45 cases of bladder cancer in this study.All patients were definitely diagnosed by historical results.Routine MRI and DWI were performed on each patient before operation.ADC map was generated in the workstation while ADC value of tumor were measured at b-factor of 800 s/mm2.The expressions of PCNA and Ki-67 were detected by immunohistochemical(SP) staining.Relationship between ADC value and the expressions of PCNA and Ki-67 in bladder cancer was then analyzed.Results A reverse trend was found in ADC value and expression of PCNA and Ki-67 in bladder cancer.The higher expression of PCNA and Ki-67,the lower value ADC was.ADC value of all tumor in high expression of PCNA and Ki-67 was(0.79±0.12)×10-3 s/mm2 and(0.77±0.10)×10-3 s/mm2,respectively.However,ADC value in low expression group was(1.12±0.15)×10-3s/mm2 and(1.05±0.08)×10-3 s/mm2,respectively.There was significantly negative correlation coefficients between the ADC value with the expressions of PCNA and Ki-67 of bladder cancer(r=-0.832,r=-0.741,P<0.05).Conclusion The ADC value of bladder cancer reflect the degree of cell proliferation at molecule level,the measurement of ADC value play an important role in diagnosis of the malignant level and prognosis of bladder cancer befor operation.
