【摘要】 目的 探讨软组织脂肪肉瘤不同亚型的CT表现特点。方法 回顾性分析15例、17例次经手术及病理证实的脂肪肉瘤的CT表现,并分析其与病理分型之间的关系。17例次中,腹膜后脂肪肉瘤11例,为高分化型2例,黏液型3例,圆形细胞型1例,去分化型1例,混合型4例;腹腔2例,为高分化型和去分化型;腕部1例,为黏液型;大腿1例,为高分化型;胸壁2例,为黏液型和多形性。结果 高分化型脂肪肉瘤4例均表现为脂肪密度为主型肿块,增强后轻度强化。黏液型5例均表现为密度介于水样与软组织密度间的肿块,2例增强扫描分别呈不均匀中度强化和絮状轻度强化。圆形细胞型及多形性各1例为无脂肪密度的软组织肿块。去分化型2例以脂肪密度为主,内见软组织肿块,强化CT值最大增加达100 HU。混合型4例可见上述各型脂肪肉瘤表现。结论 软组织脂肪肉瘤组织学的多样性决定了CT表现的多样性,了解这些CT表现特点有助于脂肪肉瘤的术前诊断及分型.。更多还原

【Abstract】 Objective To explore the CT features of liposarcoma with different pathological subtypes.Methods Fifteen cases with liposarcoma proved by operation and pathology underwent 17 operations and the data from 17 treatments were retrospectively analyzed.Imaging features were analyzed and compared with different histological subtypes.These 17 cases included 11 cases of retroperitoneal liposarcoma of which the pathological subtypes included 2 cases of well-differentiated liposarcoma,3 cases of myxoid liposarcoma,1 case of round-celled liposarcoma,1 case of dedifferentiated liposarcoma and 4 cases of mixed liposarcoma.Two cases with intraperitoneal liposarcoma have been observed to be the well-differentiated liposarcoma and dedifferentiated liposarcoma,while 1 case with wrist liposarcoma was myxoid liposarcoma;1 case of thigh was well-differentiated liposarcoma,and 2 cases of chest were myxoid liposarcoma and pleomorphic liposarcoma.Results On CT image,well-differentiated liposarcoma showed masses with fatdensity and fewer enhancement.Myxoid liposarcoma presented low density,and two of the 5 lesions showed enhancement with inhomogeneous and flocculent strengthen.Round-celled liposarcoma and pleomorphic liposarcoma displayed as soft tissue mass containing no fat.Dedifferentiated liposarcoma was demonstrated as fat-density containing soft tissue,and the enhancement CT density could increase 100HU at maximum.The mixed-type liposarcoma consisted of the above appearances.Conclusion Varieties of CT features of liposarcomma are attributed to their histological subtypes.Understanding the liposarcoma ’s CT features is helpful for the preoperative diagnosis and classification of the disease.更多还原
