慢性阻塞性肺疾病患者C-反应蛋白与降钙素原水平的变化与意义
Changes and Significance of C-reactive Protein and Procalcitonin Level in Patients with Chronic Obstructive Pulmonary Disease
【摘要】 目的 探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者C-反应蛋白(C-reactive protein,CRP)及降钙素原(procalcitonin,PCT)水平变化,评估CRP及PCT水平检测的临床意义。方法 纳入2012年2月~2013年2月132例COPD患者,分别于急性加重期及稳定期测定CRP及PCT的水平并进行分析;分析不同肺功能损害程度患者的CRP水平;分析单纯COPD患者及COPD伴合并症患者CPR水平。评估CRP,PCT水平对于COPD患者诊断及病情监测的临床意义。结果 COPD急性加重期CRP及PCT水平显著高于稳定期(CRP水平分别为58.24±9.24 mg/L和11.22±7.14 mg/L,t=3.25,P<0.05;PCT水平分别为1.45±0.35 ng/ml和0.07±0.03 ng/ml,t=4.42,P<0.05);PCT水平在COPD稳定期降至正常,而CRP仍维持较高水平。CPR水平在GOLD 4级肺功能患者显著高于GOLD 1级肺功能患者(GOLD 4级:n=29,CRP水平10.83±4.52 mg/L;GOLD 1级:n=35,CRP水平6.85±2.15 mg/L;t=3.27,P<0.05);CRP水平在COPD伴并发症患者较单纯COPD患者显著升高(COPD伴并发症患者:n=70,CRP水平11.24±4.23 mg/L;单纯COPD患者:n=62,CRP水平7.52±3.11 mg/L;t=4.22,P<0.05)。结论 CRP,PCT水平与COPD患者急性加重相关,CRP及PCT水平检测可作为COPD诊断及病情监测的有效标志物。
【Abstract】 Objective To observe the C-reactive protein(CRP) and procalcitonin(DCT) level changes to evaluate clinical diagnosis value in patients with chronic obstructive pulmonary disease(COPD).Methods In 2012 February to 2013 February 132 cases of patients with COPD,respectively in acute exacerbation period and stable period to determine the CRP and PCT levels and analysed of different pulmonary function damage in patients with CRP level.Analysed CRP level of simple COPD patients and COPD with complications patients with CPR.Evaluation of CRP,PCT level for clinical significance diagnosis and condition monitoring in patients with COPD.Results Acute exacerbation of COPD CRP and PCT were significantly higher than those in stable period(CRP 58.24±9.24 mg/L,ll.22±7.14 mg/L,t = 3.25,P<0.05;PCT 1.45±0.35 ng/ ml.0.07±0.03 ng/ml,t=4.42,P<0.05).The level of PCT decreased to normal in stable phase of COPD,while CRP maintained high level.CPR level was significantly higher in patients with GOLD 4 lung pulmonary function than that of GOLD 1 grade.(GOLD 4 grade:n=29,CRP level 10.83±4.52 mg/L;GOLD 1 grade:n=35,CRP level 6.85 + 2.15 mg/L;t=3.27, P<0.05);CRP level in COPD patients with comorbidities than patients with simple COPD significantly increased(COPD patients with complications:n= 70,CRP level 11.24 ±4.23 mg/L;patients with simple COPD:n = 62,CRP level 7.52 ± 3.11 mg/L;t=4.22,P<0.05).Conclusion The level of CRP,PCT level related to the acute exacerbation in patients with COPD,detection of CRP and PCT levels can be used as an effective biomarker for COPD diagnosis and disease monitoring.
