碳青霉烯类抗菌药物敏感性降低的肺炎克雷伯菌流行病学分析及耐药性观察

【摘要】 目的观察医院碳青霉烯类抗菌药物敏感性降低的肺炎克雷伯菌的流行病学及耐药情况。方法采用Vitek 2 Compact进行细菌鉴定及药敏试验,从我院2010年11月~2011年10月临床标本中筛选出14株碳青霉烯类抗菌药物敏感性降低的肺炎克雷伯菌,以WHONET 5.4软件统计分析菌株的流行病学资料及耐药情况。结果14株碳青霉烯类抗菌药物敏感性降低的肺炎克雷伯菌主要分离自NICU和神经科ICU患者的痰标本,患者性别无明显差异,多为新生儿及年龄>60岁的老年人,其中12例有气管插管操作;菌株均为多重耐药,且其耐药率与往年比较呈上升趋势。结论碳青霉烯类敏感性降低的肺炎克雷伯菌已在我院NICU局部流行,且均为泛耐药菌株;合理使用抗菌药物,做好重点科室多重耐药菌感染者的消毒与隔离,是控制此类感染的有效措施。

【Abstract】 Objective To investigate the epidemiology and resistance to antibiotics of carbapenem-non-susceptible Klebsiella pneumoniae in our hospital.Methods Using Vitek 2 Compact for bacterial identification and susceptibility testing,fourteen carbapenem-non-susceptible Klebsiella pneumoniae strains were collected in our hospital from November 2010 to October 2011.The epidemiology and resistance to antibiotics was analyzed by WHONET 5.4.Results Fourteen carbapenem-non-susceptible Klebsiella pneumoniae were mainly isolated from the specimens of intensive care unit(ICU) and neonatal intensive care unit(NICU),no significant difference in patients’ gender,the patients were mainly newborns and the elderly over the age of 60 years,12 patients were operated intubation;All stains were multi-drug resistant,and there was a yearly upward trend.Conclusion There exist a prevalence of carbapenem-non-susceptible Klebsiella pneumoniae in NICU of our hospital and these strains are pan-resistant;The effective measures to prevent infection of these stains are using antibiotics drugs reasonablely and implementing the disinfection and isolation of multi-drug resistant in key departments.
