后路开窗减压、伤椎短节段固定治疗胸腰椎爆裂性骨折效果观察

【摘要】 目的探讨后路开窗减压、伤椎和相邻节段椎弓根钉固定治疗胸腰椎爆裂性骨折的效果。方法选择35例胸腰椎爆裂性骨折患者,通过开窗减压或半椎板减压,在伤椎和正常椎体植入椎弓根钉固定,术后定期记录椎体高度、神经功能和后突角变化情况。结果 35例胸腰椎爆裂性骨折患者术后随访1~5年,伤椎骨愈合时间为2~4个月,术后6个月骨折完全愈合、神经功能明显改善、术后椎体高度未见明显丢失、Cobb’s角较术前明显改善,Frankel分级有好转。结论后路开窗减压、伤椎短节段固定治疗胸腰椎爆裂性骨折安全、有效。

【Abstract】 Objective To explore the effect of the treatment of thoracolumbar burst fractures through the posterior approach fenestration or semi-laminectomy decompression and fixation by embedding pedicle screws between the injured and normal vertebrae.Methods 35 cases of thoracolumbar burst fractures patients were selected.After fenestration or semi-laminectomy decompression,the adjacent injured-vertebrae and normal vertebras were fixed by embedding pedicle screws.In order to evaluate the safety and effectiveness of this method,the injured-vertebral heights,neurological functions and the after process angles were recorded regularly after the surgery.Results 35 cases of the patients were followed for 1 to 5 years,2 to 4 months after the surgery,the fracture recovered markedly,neurologic function improved markedly and no obvious vertebral body height loss after the restoration during a review of X-ray,Cobb′s horns angle improved obviously compared with anterior-operation,Frankel classification improved also.Conclusions Through the posterior fenestration or semi-laminectomy decompression,the treatment of thoracolumbar burst fractures through fixation by embedding pedicle screws into the injured-vertebrae and normal vertebrae is safe and effective.
