选择性头部亚低温对重度窒息新生儿脑的保护作用

【摘要】 目的探讨选择性头部亚低温对重度窒息新生儿脑的保护作用。方法将足月重度窒息新生儿62例根据治疗方法的不同分为治疗组32例与对照组30例,治疗组给予头部亚低温干预,对照组给予常规干预。结果两组7天时新生儿行为神经测定评分分别为(36.9±2.5)分和(34.9±2.3)分,两组比较差异有统计学意义(t=2.52,P<0.05);28天时新生儿行为神经测定评分分别为(39.5±1.4)分和(36.8±2.0)分,两组比较差异有统计学意义(t=3.34,P<0.05)。所有患儿得到随访,两组患儿大运动DQ值、精细运动DQ值、适应性DQ值和语言DQ值比较差异均有统计学意义(t值分别为16.33、6.52、3.25、7.00,均P<0.05)。结论选择性头部亚低温治疗对重度窒息新生儿安全、可行,而且有明显的近期和远期脑保护作用。

【Abstract】 Objective To investigate the protective effect of selective head mild hypothermia on brain of severe asphyxia neonates. Methods Totally 62 cases of full-term neonates with severe asphyxia were divided into treatment group(n=32) and control group(n= 30) according to different treatment.The treatment group was given the head mild hypothermia intervention,and the control group received routine intervention.Results The NBNA scores of two groups at 7d were 36.9±2.5 and 34.9±2.3,and there was significant difference(t=2.52,P<0.05).The scores at 28d were 39.5±1.4 and 36.8±2.0,and the difference was significant(t=3.34,P<0.05).All of the patients were followed,and there were statistical differences in sports DQ value,fine motor DQ value,adaptive DQ value and language DQ value between two groups(t value was 16.33,6.52,3.25 and 7.00,respectively,all P<0.05).Conclusion Selective head hypothermia therapy is security and viable for neonatal asphyxia,and it has significant near-term and long-term brain protection.
