【摘要】 目的探讨2型糖尿病并脂肪肝患者血浆碱性成纤维细胞生长因子21(FGF21)水平变化及其临床意义。方法随机抽取我院60例2型糖尿病并脂肪肝,50例单纯脂肪肝患者和50例健康体检者(健康组)进行对比研究。检测人碱性成纤维细胞生长因子21(FGF21)、肝功能(ALT,AST)、胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)和游离脂肪酸(FFA)等生化指标,体重指数(BMI)、腰臀比(WHR)、空腹血糖(FBG)、空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)等相关指标。结果 (1)2型糖尿病并脂肪肝患者与健康组相比,ALT、AST、TC、TG、LDL-C、FFA、BMI、WHR、FBG、FINS、HOMA-IR和FGF21均显著增高(P<0.05),HDL-C降低(P<0.05);(2)2型糖尿病并脂肪肝组与单纯脂肪肝组相比,TG、FFA、FBG、FINS、HOMA-IR和FGF21均显著增高(P<0.05),BMI、WHR明显降低(P<0.05);(3)血浆FGF21水平与TG、FFA、BMI、HOMA-IR呈正相关(P<0.05)。结论 2型糖尿病并脂肪肝患者血浆FGF21水平升高,FGF21可能与肥胖、脂代谢紊乱、IR有关,可能是2型糖尿病并脂肪肝发病的重要危险因素,对2型糖尿病并脂肪肝发生、发展起重要作用。更多还原

【Abstract】 Objective To discuss the levels of serum FGF21 in patients with type II diabetes complicated fatty liver and its clinical significance.Methods Sixty cases patients with type II diabetes complicated fatty liver were extracted randomly as.Fifty cases patients with only fatty liver and fifty normal guys were selected as control group.To detect levels of serum FGF21,ALT,AST,TC,BMI,WHR,FBG,FINS and HOMA-IR in these cases.Results(1) Levels of serum ALT,AST,TC,TG,LDL-C,FFA,BMI,WHR,FBG,FINS,HOMA-IR and FGF21 were both obviously increase in experimental group and its in normal group(P < 0.05);(2) Levels of serum of TG,FFA,FBG,FINS,HOMA-IR,and FGF21 were both significantly increased in experimental group and the only fatty liver group(P < 0.05),while the levels of BMI,WHR in serum were significantly decreased in experimental group(P < 0.05).Conclusions The levels of serum FGF21 increase in patients with type II diabetes complicated fatty liver.FGF21,which may be related to fat,metabolic disorder,and IR,perhaps is an important risk factor in the process of type II diabetes complicated fatty liver.It can take into effect to the occurrence and development of type II diabetes complicated fatty liver.更多还原
