【摘要】 目的探讨后腹腔镜下肾蒂淋巴管剥脱术在乳糜尿治疗中的临床价值。方法确诊乳糜尿患者12例。男8例,女4例,年龄36～72岁。左侧10例,右侧2例。均行后腹腔镜下肾蒂淋巴管剥脱术,无中转开放手术。术中使用超声刀游离,较大淋巴管钛夹夹闭,留置尿管和肾周引流管。结果 12例手术均较顺利完成,手术时间160～240 min,平均180 min左右;估计出血量80～200 ml,平均110 ml,平均住院9 d。术后随访3～12个月,除2例偶有尿液微混,尿乳糜试验阳性外,其余10例均未复发。结论后腹腔镜下肾蒂淋巴管剥脱术具有创伤小、术中出血少、术后恢复快、疗效确切特点,是治疗严重乳糜尿的有效方法。更多还原

【Abstract】 Objective To observe the clinical value of retroperitoneoscopic renal pedicle lymphatic disconnection in the management of chyluria.Methods Twelve cases of chyluria(8 men and 4 women,age ranged from 36 to 72 years old) were treated with laparoscopic renal lymphatic stripping,10cases with lesion on the left side,2 on the right.The ultrasonic harmonic scalpel was used during the dissociation and ligation.Results Operations were succeeded in all patients.The average operation time was 180 minutes,blood loss 110ml,hospitalizing time 9 days.No recurrence of chyluria was found during the follow-up of 3~12 months.Two patients with bilateral chyluria occationally showed chyluria during follow-up.Conclusions Retroperitoneoscopic renal pedicle lymphatic disconnection for chyluria is an effective and efficient method in the management of chyluria.And its minimal invasive,less blood loss,short hospital stay and rapid recovery of gastrointestinal function.更多还原
