鼻咽癌组织DNA-PKcs和BRCA1蛋白表达临床意义分析
Expressions of DNA-PKcs and BRCA1 and their clinical significance in nasopharyngeal carcinoma tissues
【摘要】 目的:研究鼻咽癌组织中DNA依赖蛋白激酶催化亚基(DNA-dependent protein kinase catalytic subunit,DNA-PKcs)和乳腺癌基因1(breast cancer 1,BRCA1)的蛋白表达及其临床意义。方法:收集2007-05-07-2011-11-23广西壮族自治区人民医院病理学确诊为鼻咽癌且应用调强放射治疗(intensity modulated radiation therapy,IMRT)的患者71例。采用免疫组化SP法检测71例患者中DNA-PKcs及BRCA1在鼻咽癌组织中的表达。阳性细胞计数≥50%分别定义为DNA-PKcs或BRCA1高表达组;阳性细胞计数<50%及不表达者分别定义为DNA-PKcs或BRCA1低表达组。并结合其临床资料,分析2种蛋白的表达与患者的急性放射反应、临床分期、肿瘤消退及生存时间的关系。结果:DNA-PKcs的表达与患者的AJCC分期(P=0.560)、皮肤(P=0.314)、黏膜(P=0.509)、涎腺急性放射损伤(P=0.403)及肿瘤消退情况(P=0.480)无显著相关性。BRCA1的表达与患者的AJCC分期(P=0.537)、皮肤(P=0.943)、黏膜(P=0.466)、涎腺急性放射损伤(P=0.651)及肿瘤消退情况(P=0.540)无显著相关性。DNA-PKcs表达与患者的生存时间呈正相关,r=0.234,P=0.048。结论:鼻咽癌组织中DNA-PKcs和BRCA1的表达与患者的临床分期、放射性损伤以及肿瘤的消退情况无关。DNA-PKcs低表达可能成为预测鼻咽癌患者预后较差的一个指标。
【Abstract】 OBJECTIVE:To investigate the expressions of DNA-dependent protein kinase catalytic subunit(DNAPKcs)and breast cancer 1(BRCA1)in nasopharyngeal carcinoma(NPC)and to analyze the clinical interrelation.METHODS:Between 7May,2007and 23November,2011all 71untreated patients with NPC,diagnosed pathologically and received intensity modulated radiation therapy(IMRT)at the People’s Hospital of Guangxi Autonomous Region were enrolled in this study.The expression of DNA-PKcs and BRCA1proteins in the tissue of nasopharyngeal carcinoma was detected by immunohistochemistry.The overexpression of DNA-PKcs or BRCA1groups included patients whose biopsy specimens showed at least 50% immunopositive tumor cells.Patients who had less than 50% of the tumor cells were placed in the low DNA-PKcs and BRCA1groups.The relationship between the expression of the two proteins and the TNM classification,acute radiation injury,tumor regression,and survival time of the patients was annlyzed.RESULTS:No significant differences were found between the expression of the DNA-PKcs proteins and the AJCC classification(P= 0.560),acute radiation injury of skin(P=0.314),mucous membranes(P=0.509),salivary gland(P=0.403)and tumor regression(P=0.480).No significant differences were found between the expression of the BRCA1proteins and the AJCC classification(P=0.537),acute radiation injury of skin(P=0.943),mucous membranes(P=0.466),salivary gland(P= 0.651)and tumor regression(P=0.540).There was a significant positive correlation between the expression leval of DNA-PKcs and survival time of patients,r=0.234,P=0.048.CONCLUSIONS:There is no significant correlation between the expression level of DNA-PKcs or BRCA1and the TNM classification,acute radiation injury and tumor regression in nasopharyngeal carcinoma.Low expression of DNA-PKcs can be act as an indicator of poor prognosis in patients with nasopharyngeal carcinoma.
