【摘要】 目的探讨行血液透析(HD)的糖尿病肾病(DN)患者肺炎的发病情况及死亡相关危险因素。方法选择2007年1月—2012年1月在广州军区广州总医院肾内科血液净化中心治疗的终末期肾病患者1 145例,根据是否合并DN,分为DN组190例及non-DN组955例。经规律HD后对两组患者的肺炎发生情况、死亡危险因素及生存情况进行比较。结果 DN组患者的肺炎发生率高于non-DN组患者〔12.6%(24/190)与5.6%(54/955)〕,差异有统计学意义(χ2=12.152,P<0.05)。Cox回归分析显示,年龄增加是肺炎患者死亡的独立危险因素。KaplanMeier累积生存时间分析曲线显示,DN组1、3、5年累积全因死亡率和累积肺炎相关死亡率均高于non-DN组(P<0.05)。结论合并DN的HD患者更容易发生肺炎,其因肺炎死亡的风险也明显高于non-DN的HD患者,所以合并DN的HD患者加强治疗护理显得极其重要。更多还原

【Abstract】 Objective To explore the occurrence of pneumonia and risk factors related to death in post- hemodialysis( HD) patients with diabetic nephropathy( DN). Methods A total of end- stage renal disease( ESRD) patients hospitalized in this hospital from January 2007 to January 2012 were divided,according whether combined with DN or not,into groups DN( n =190),non- DN( n =955). After regular HD,the occurrence of pneumonia,death risk factors and survivals were analyzed comparatively.
