应用非骨水泥型假体进行全髋关节置换术治疗类风湿性关节炎的早期并发症研究
Early Surgical Complications of Cementless Total Hip Arthroplasty in the Treatment of Rheumatoid Arthritis
【摘要】 目的探讨应用非骨水泥型假体行全髋关节置换术(THA)治疗类风湿关节炎(RA)的早期手术并发症。方法回顾性分析天津市天津医院关节外科2003年4月—2010年5月收治的34例(36髋)应用非骨水泥型假体行THA的RA患者的临床资料,研究RA患者行THA的手术并发症如术中假体周围骨折、术后假体移位和早期松动以及无菌性松动的发生率。结果 36髋应用非骨水泥型假体行THA,术中髋臼骨折1例,骨折的发生率为2.8%,属于Peterson和Lewallen骨折分型的Ⅰ型骨折,术后早期未出现髋臼假体移位、早期松动及无菌性松动。未出现术中股骨骨折,随访时未出现股骨假体下沉和无菌性松动。结论 RA患者应用非骨水泥型假体行THA术后,很少出现术中假体周围骨折,假体的骨整合和初始稳定性良好,假体移位和早期松动以及无菌性松动的发生率未增加。
【Abstract】 Objective To explore the early surgical complications of cementless total hip arthroplasty( THA) in the treatment of rheumatoid arthritis( RA). Methods From April 2003 to May 2010,34 RA patients( 36 hips) were treated with cementless THA at the department of joint surgery,Tianjin Hospital,the clinical data of whom were evaluated retrospectively. The rates of the surgical complications such as intra- operative fractures around cementless,postoperative prosthesis migration,early and aseptic loosening were calculated after surgery. Results There was one case of acetabular fracture among 34 RA patients( 36 hips) who were treated with cementless THA,the fracture could be classified as type Ⅰ according to the Peterson and Lewallen classification,and the fracture rate was 2. 8%. There was no postoperative prosthesis migration,early and aseptic loosening in the immediate postoperative period. There was no intra- operative femoral fracture. There was no subsidence and aseptic loosening during the follow- up period. Conclusion Among RA patients who had cementless THA,seldom cases suffered from intra- operative fractures around cementless,the initial implant stability and osseous integration are satisfactory,and the incidence rates of postoperative prosthesis migration,early loosening and aseptic loosening did not increase.
