【摘要】 目的探讨彩色多普勒超声结合踝、趾臂指数在诊断2型糖尿病(T2DM)下肢中小型动脉病变的价值。方法应用彩超及全自动动脉硬化检测仪对129例T2DM患者的腘动脉、胫前动脉、胫后动脉及足背动脉进行检查,按病程长短及是否有合并症分为单纯糖尿病组与糖尿病合并症组,比较2组动脉硬化发生情况与ABI、TBI关系及下肢中小动脉粥样硬化部位发生率。结果 129例T2DM检查出动脉粥样硬化116例,阳性率为89.9%;动脉粥样硬化发病率最高为胫前动脉,狭窄及闭塞发生率也同前。随着T2DM病程的延长及合并症增加,ABI、TBI值下降及下肢动脉粥样硬化发生率增高、程度加重。结论彩色多普勒超声结合踝、趾臂指数能反映2型糖尿病患者下肢动脉病变,对糖尿病患者下肢动脉病变的早期诊断、预防坏疽的发生有临床价值。更多还原

【Abstract】 Objective To study the diagnosis of small and medium arteries of lower limb in type 2 diabetes mellitus( T2DM) by using color Doper imaging and toe- brachial index. Methods Ultrasound detector and automatic atherosclerosis was used in 129 patients with T2DM for detecting popliteal artery,anterior tibial artery,posterior tibial artery and dorsalis pedis artery. And check whether according to duration of disease and diabetes with complications were divided into three groups with diabetic complications group were compared with the incidence of atherosclerosis ABI,TBI relations and lower extremity atherosclerosis parts small incidence. Results In 129 cases of T2DM,there were 116 cases check out atherosclerosis( 89. 9%). Atherosclerosis,stenosis and occlusion were the highest incidence of anterior tibial artery. With the extension of the course and complications of T2DM increased,ABI,TBI value decreases and lower extremity atherosclerosis increased incidence and degree aggravated. Conclusion Color Doppler ultrasound combined with ankle, toe- brachial index in diagnosis of patients with type 2 diabetes with in the early diagnosis,prevention of the occurrence of gangrene have important clinical significance.更多还原
