降钙素原、C反应蛋白与急性胰腺炎严重程度和预后的关系
Relationship between serum procalcitonin,C reactive protein and severity and prognosis of acute pancreatitis
【摘要】 目的探讨降钙素原(PCT)、C反应蛋白(CRP)与急性胰腺炎(AP)严重程度和预后的相关性。方法回顾性分析106例急性胰腺炎(AP)的降钙素原、C反应蛋白水平,其中重症急性胰腺炎(SAP)45例,轻症急性胰腺炎(MAP)61例。分析它们与其他生化指标、AP病情程度及患者预后的相关性。结果 SAP组患者血清PCT及CRP水平分别为(7.86±2.38)μg/L和(148.26±16.46)mg/L,明显高于MAP组的(5.22±2.13)μg/L和(47.62±9.89)mg/L(P<0.01)。AP患者血清PCT水平与CRP水平呈正相关(r=0.316,P<0.01),且两者与24 h APACHE II评分、Ranson评分、Balthazar CT评分均呈正相关(P<0.05或<0.01)。结论检测PCT与CRP水平有助于判断AP病情发展和预测预后。
【Abstract】 Objective To analyze the correlation of serum procalcitonin( PCT),C reactive protein( CRP)and severity and prognosis of acute pancreatitis( AP). Methods The serum levels of PCT,CRP of 106 cases of acute pancreatitis were collected and analyzed retrospectively,including 45 severe acute pancreatitis( SAP) and 61 mild acute pancreatitis( MAP). Then their relationship with other biochemical parameters,the severity and prognosis of AP was analyzed. Results The serum PCT and CRP levels were( 7. 86 ± 2. 38) μg / L and( 148. 26 ± 16. 46) mg / L in SAP group,and both were significantly higher than those in MAP( 5. 22 ± 2. 13) μg / L and( 47. 62 ± 9. 89) mg / L( P <0. 01). The serum levels of PCT was positively correlated with serum levels of CRP( r = 0. 316,P < 0. 01),and both were positively correlated with 24 h APACHE IIscore,Ranson score,Balthazar CT score( P < 0. 05 or < 0. 01). Conclusion Our results indicate PCT and CRP levels may evaluate progression of AP and predict prognosis at an early date.
