微柱凝胶技术在新生儿ABO溶血病检测中的应用

【摘要】 目的探讨微柱凝胶技术在新生儿ABO溶血病(ABO-HDN)筛查中的应用。方法采用微柱凝胶技术对疑为ABO型新生儿溶血病的580例黄疸患儿做直接抗人球蛋白试验、游离试验、放散试验。结果 ABO-HDN检出率为17.6%(102/580),在102例ABO-HDN的患儿中,A型56例,阳性率为18.0%(56/311),B型46例,阳性率为17.1%(46/269),两组比较显示ABO-HDN的发生率与新生儿A或B型的型别差异无统计学意义(P>0.05);三项试验的检出敏感性,DTA最低为38.2%,放散试验最高为100.0%。出生天数≤3的新生儿ABO-HDN检出率22.9%,明显高于3<n≤7的新生儿检出率10.5%和>7的检出率(3.9%)。结论 MGT具有操作简单、使用方便、标本用量少、敏感性高和特异性强、阴阳性结果明显、结果易保存等优点。可为HDN提供可靠诊断依据,值得临床推广应用。

【Abstract】 Objective To investigate and analyze the application of microcolumn gel technique in screening ABO hemolytic disease of the newborn(ABO-HDN).Methods Direct antiglobulin test,antibody release test and free antibody test were performed in 580 cases suspected of ABO-NHD by using microcolumn gel assay.Results The total positive rate for ABO-HDN was 17.6%(102/580).In 102 ABO-NHD cases,56 cases(18.0%) was A blood type and 46 cases was B blood type.There was no significant difference in the distribution rates of A blood type and B blood type(17.1%) in ABO-NHD cases(P>0.05).Moreover,the sensitivity of direct antiglobulin test was the lowest,while the sensitivity of the elution test was the highest.The detection rate of ABO-HDN in newborns aged less than or equal to 3 days was higher than that in those aged greater than 3 days and less than/equal to 7 days(22.9% vs 10.5%) and those aged greater than 7 days(22.9% vs 3.9%).Conclusions With the advantages of simple to handle,easy to use,less specimen usage,high sensitivity and specificity,obvious results of negative and positive and easy to preserve the results,MGT can provide reliable basis for HDN diagnosis,it is worth popularizing.
