【摘要】 目的:建立中医综合疗效标准模型,与WHO实体瘤疗效标准在中医及中西医结合治疗晚期胃癌中的预后作用进行比较。方法:采用回顾性病例研究方法,将影响晚期胃癌预后的多个因素进行Cox回归分析,建立晚期胃癌中医综合疗效标准模型,用Kaplan-Meier法判断两种疗效标准对晚期胃癌的预后作用。结果:104例晚期胃癌患者纳入研究,根据Cox多因素回归分析,中医综合疗效标准的评价指标为瘤体大小、Karnofsky评分、体重及主症变化4项。中医综合疗效标准评价为有效与恶化的中位生存期分别为16.00与5.00个月,差异有统计学意义(χ2=72.10,P=0.000),而以WHO疗效标准评价,CR、PR、SD生存曲线相互交叉。结论:中医综合疗效标准用于评价中医及中西医结合治疗晚期胃癌的预后作用优于WHO疗效标准。更多还原

【Abstract】 Objective:The comprehensive TCM response criteria was built up.It was compared with WHO solid tumors response criteria in evaluating the efficacy in advanced gastric cancer treated with TCM or TCM combined with western medicine.Methods:Using a retrospective case study method,the Cox regression analysis was used to analyze the clinical factors that affect the prognosis of advanced gastric cancer,then the comprehensive TCM response criteria was built up.Kaplan-Meier survival analysis was used to compare two criteria for advanced gastric cancer prognosis.Results:104 patients with advanced gastric cancer were included in the study.According to Cox regression analysis,changes in tumor size,KPS,weight and main symptoms were the four factors included in the comprehensive TCM response criteria.Evaluating with the TCM criteria,the survival curve of effective cases was significantly different from that of progressive cases(MST 16.00 vs 5.00,χ2=72.10,P=0.000).But the CR,PR and SD survival curves crossed over each other in evaluating with WHO criteria.Conclusion:The TCM criteria can better reflect the prognosis of advanced gastric cancer treated by TCM or TCM combined with western medicine than the WHO criteria.更多还原
