高渗氯化钠羟乙基淀粉40预防腰硬联合麻醉后低血压
Preventive Effect of Hypertonic Saline Hydroxyethyl Starch 40 Solution on Hypotension after Combined Spinal Epidural Anesthesia
【摘要】 目的:研究5%高渗氯化钠羟乙基淀粉40(hypertonic saline hydroxyethyl starch 40,HSH40)注射液对腰硬联合麻醉时低血压的预防效果。方法:拟行子宫切除术患者60例,随机分为两组,每组30例。两组患者均行腰硬联合麻醉(combined spinal epidural anesthesia,CSEA),穿刺点为L2~3间隙。腰麻注药之前Ⅰ组患者静脉滴注HSH40注射液(4 mL/kg),之后再静脉滴注乳酸钠林格液,Ⅱ组仅静脉滴注(10 mL/kg)乳酸钠林格液。记录CSEA后10 min、20 min收缩压(SBP)及术中麻黄素用量,并检测患者手术前后血清电解质及凝血功能指标。结果:Ⅱ组患者在麻醉后10 min、20 min血压较术前显著降低,两组患者术后血压差异有统计学意义(P<0.05),Ⅱ组患者术中麻黄素的用量显著多于I组(P<0).01)。输注液体前后血清电解质及凝血功能组间差异无统计学意义(P>0.05)。结论:应用HSH40注射液能有效预防CSEA引起的低血压。
【Abstract】 Objective:To explore the preventive effect of hypertonic saline hydroxyethyl starch 40(HSH40) solution on hypotension after combined spinal epidural anesthesia.Methods:A total of 60 patients undergoing abdominal hysterectomy with combined spinal epidural anesthesia(CSEA) were randomly divided into two groups with 30 patients in each group.Before combined spinal epidural anesthesia.HSH40 solution was injected a dose of 4 mL/kg in Group I,and sodium lactate ringer’s injection was injected in a dose of 10 mL/kg in Group II,or sodium lacpate ringer’s inpectior was infected only(Group II).Systolic blood pressure(SBP) and the total amount of ephedrine used intra-operation were measured 10 min and 20 min after CSEA.The serum electrolyte and the blood coagulation function indexes were measured after CSEA in the two groups.Results:SBP was significantly lower in Group II after CSEA than before operation.There was significant difference in blood pressure between the two groups(P<0.05).The total amount of ephedrine used intra-operation was larger than that in Group I,while there were no significant differences in serum electrolytes and blood coagulation function between the two groups(P>0.05).Conclusions:Application of HSH40 solution can effectively prevent hypotension induced by CSEA.
