【摘要】 目的:探讨lumican基因在胃癌组织中的表达及其与预后的关系。方法:采用免疫组化方法检测144例胃腺癌组织及其癌旁正常黏膜组织中lumican基因的表达情况;提取40例患者新鲜胃癌组织及对应癌旁正常黏膜组织的总RNA,逆转录成cDNA,采用实时荧光定量PCR方法检测lumican基因的表达情况,分析其表达与临床各指标的关系。结果:lumican在胃癌组织的表达率显著高于癌旁正常黏膜组织(89.6%比8.3%,P<0.01),其表达与患者性别、年龄、淋巴结转移、远处转移、脉管侵润、癌栓无相关性(P>0.05)。与浸润深度(P<0.01)、肿瘤直径(P<0.05)、肿瘤部位(P<0.05)和UICC分期(P<0.05)明显相关。胃癌组织中lumican mRNA表达水平显著高于癌旁正常黏膜组织[(2.58±1.62)比(3.95±1.34)],差异有统计学意义(P<0.05)。lumican表达阴性组、弱阳性组和强阳性组胃癌患者的术后5年总体生存率分别为86.7%、46.9%和30.9%,组间差异有统计学意义(χ2=16.927,P<0.01)。多因素COX模型分析显示,lumican表达、远处转移、肿瘤直径、国际抗癌联盟(UICC)分期和脉管侵犯是影响总体生存的独立预后因素。与lumican表达阴性组比较,强阳性组和弱阳性组有增加胃癌病死风险的趋势;强阳性组、弱阳性组与阴性组比较,强阳性组RR=4.972(95%CI:1.110～20.822)、弱阳性组RR=5.910(95%CI:1.399～24.922)。结论:lumican表达水平与胃癌增殖、侵袭能力、病理分期及总体生存率密切相关,对判断胃癌患者预后有一定的临床参考意义。更多还原

【Abstract】 Objective:To explore the expression level of lumican in gastric cancer tissues and its correlation with prognosis.Methods:The expression of lumican was detected in gastric cancer tissues and the corresponding paracancerous tissues of 144 patients with gastric cancer by immunohistochemistry assay.Total RNA extracted from gastric cancer tissues and adjacent normal tissues of 40 patients with gastric cancer was reverse-transcripted into cDNA.Real-time fluorescent quantitative PCR was used to analyze the expression level of lumican gene.The relationship between the expression level of lumican and different cli-nical indexes was analyzed.Results: The expression rate of lumican in gastric cancer tissue was higher than that in the corresponding paracancerous tissue(89.6 % vs.8.3 %,P<0.01).The expression of lumican was not correlated with gender,age,lymph node metastasis,distant metastasis,vascular invasion and tumor thrombus(P>0.05).However,significant correlations were found in the expression of lumican with invasion depth(P<0.01),tumor size(P<0.05),tumor location(P<0.05) and clinical stage(P<0.05).The expression of lumican mRNA was higher in gastric cancer tissue than that in the adjacent normal tissue(2.58±1.62) vs.(3.95±1.34),P<0.05.The overall survival rates of patients with gastric cancer after five years in lumican expression negative group,weak-positive group and strong-positive group were 86.7 %,46.9 % and 30.9 %,respectively(χ2=16.927,P<0.01).Multivariate COX-model analysis showed that expression of lumican,distant metastasis,tumor size,UICC stage and vascular invasion were independent prognostic factors of overall survival.Risk ratios(RR) in strong-positive group were 4.972(95 % CI: 1.110~20.822)and 5.910(95 % CI: 1.399~24.922) in weak-positive group,compared with negative group.Conclusions: Lumican may play important roles in the process of gastric cancer proliferation and invasion.更多还原
