【摘要】 目的 探讨细胞角蛋白5/6(Cytkoeratin 5/6,CK5/6)和细胞角蛋白7(Cytkoeratin 7,CK7)在非小细胞肺癌(Non-small cell lung cancer,NSCLC)中的表达对NSCLC诊断和鉴别诊断的价值。方法 收集1067例NSCL患者的病理组织标本及相关临床资料,采用免疫组化EnVision两步法分别检测CK5/6和CK7在NSCLC中的表达并对结果进行分析。结果 CK5/6和CK7在NSCLC中的阳性表达率分别为47.0%(233/496)和72.1%(401/556)。CK5/6主要表达于肺鳞癌(162/180),其诊断敏感性和特异性分别为96.4%和89.0%,准确度为92.8%。CK7主要表达于肺腺癌(217/223),其诊断敏感性为97.3%,特异性为69.4%,准确度85.8%。两者联合检测共379例,CK5/6(+)/CK7(-)110例,以鳞癌为多,占此种表达的98.2%;CK5/6(-)/CK7(+)191例,以腺癌居多,占此种表达的94.8%。CK5/6和CK7的阳性表达呈负相关(r=-0.668,P<0.001)。结论 在NSCLC中CK5/6和CK7分别是鳞癌和腺癌的重要标志物,联合检测大大提高了NSCLC诊断和鉴别诊断的准确度。更多还原

【Abstract】 Objective To investigate the value of cytkoeratin 5/6(CK5/6) and cytkoeratin 7(CK7) in the diagnosis and differential diagnosis for non-small cell lung cancer(NSCLC).Methods The clinical data and pathological specimens from 1067 NSCLC were detected by immunohistochemistry.The EnVision two-step method was applied for detecting and analysing CK5/6 and CK7 expression in NSCLC tissues.Results The positive rates of CK5/6 and CK7 in NSCLC tissues were 47.0%(233/496) and 72.1%(401/556),respectively.The expression of CK5/6 was more frequent in squamous cell carcinoma(SCC)(162/180) with a sensitivity of 96.4%and a specificity of 89.0%and an accuracy of 92.8%.The accuracy of CK7 was 85.8%,and its sensitivity and specificity to adenocarcinoma(AdC) were 97.3%and 69.4%,respectively.The combined detection of CK5/6 and CK7 was applied in 379 cases,98.2%(110 cases) of those who represented CK5/6(+)/CK7(-)were SCC,while 94.8%(191 cases)of CK5/6(-)/CK7(+) were AdC.The CK7 expression was negatively correlated with CK5/6(r=-0.668,P <0.001).Conclusion CK5/6 and CK7 might be effective bio-markers for SCC and AdC in NSCLC.The combined detection of them may greatly improve the accuracy of diagnosis and identification for NSCLC.更多还原
