【摘要】 目的调查上海城区老年人群中痴呆的患病率。方法采用横断面调查,对≥60岁的社区居民3 141名进行了全套神经系统体检、神经心理测试和问卷调查,根据国际统一的诊断标准,DSM-Ⅳ标准诊断痴呆,NINCDS-ADRDA和NINDS-AIREN标准诊断阿尔茨海默病(AD)和血管性痴呆(VaD)并计算人群痴呆的患病率,以及各亚型构成比和患病率。结果诊断痴呆156例,痴呆患病率为5.0%;女性痴呆的患病率为5.8%,显著高于男性(4.0%,P=0.012)。痴呆患病率随年龄的增长有显著升高趋势,随文化程度的提高有显著下降趋势(P<0.001)。AD、VaD、其他类型痴呆和未分型痴呆的比例分别为72.4%、16.0%、5.1%和6.5%;女性AD患病率为4.7%,显著高于男性(2.3%,P<0.001)。AD和VaD的患病率均随年龄的增长而升高(P<0.001)。结论上海城区老年人群中痴呆患病率为5.0%,其中以AD(72.4%)为痴呆的主要类型。更多还原

【Abstract】 Aim To investigate the prevalence of dementia among elderly in an urban community in Shanghai.Methods A cross-sectional study was used.3 141 residents with 60 years old or more were interviewed with neurological examination,neuropsychological tests,and risk factor questionnaires.Diagnosis of dementia was made based on the international standard diagnosis criteria(DSM-Ⅳ for dementia,NINCDS-ADRDA and NINDS-AIREN for Alzheimer’s disease(AD) and vascular dementia(VaD).The prevalence of dementia,and the percentage and prevalence of subtypes were calculates.Results 156 cases of dementia were diagnosed.The prevalence of dementia was 5.0%.The dementia prevalence of female(5.8%) was significantly higher than that of male(4.0%,P=0.012).Prevalence of dementia significantly increased by enhanced age,and decreased by enhanced education(P<0.001).Cases with AD,VaD,other types of dementia and unknown type occupied 71.5%,15.8%,5.7% and 7.0% respectively among the dementia cases.The AD prevalence of female(4.7%) was significantly higher than that of male(2.3%,P<0.001).Prevalence of AD and VaD significantly increased by enhanced age(P<0.001).Conclusion The prevalence of dementia is 5.0% in the elderly population in urban Shanghai.The main type of dementia is AD,with the prevalence of 72.4%.更多还原
