瞬时波强技术评价早期2型糖尿病患者颈总动脉弹性功能

【摘要】 目的探讨瞬时波强(WI)技术早期评价2型糖尿病(T2DM)患者血管弹性的临床意义。方法 T2DM患者29例,其中14例病程为1~5年(A组),15例病程﹥5年(B组);30例健康志愿者为对照组。使用WI技术对各组受检者左侧颈总动脉进行检测,获得瞬时加速度波强(W1)、瞬时减速度波强(W2)、负向波面积(NA)、心电图R波顶点到W1顶点的时间(R-W1)、W1顶点到W2顶点的时间(W1-W2)、血管压力-应变弹性模量(Eρ)、血管僵硬度(β)、动脉顺应性(AC)、脉搏波速度(PWVβ)及颈动脉内-中膜厚度(IMT),结果进行对比分析。并将T2DM患者糖化血红蛋白、颈总动脉弹性参数及IMT与WI参数进行相关性分析。结果与对照组比较,A组Eρ、β及PWVβ显著升高(P﹤0.05),AC显著降低(P﹤0.01),IMT比较差异无统计学意义;B组β、Eρ、PWVβ、IMT及W2均显著升高(P﹤0.01),AC显著降低(P﹤0.01)。与A组比较,B组PWVβ和IMT显著升高(P﹤0.05)。T2DM患者W1-W2与糖化血红蛋白呈显著负相关(r=-0.56,P﹤0.01);β和IMT与年龄呈正相关(r=0.37和0.42,P﹤0.05)。结论 T2DM患者颈动脉弹性降低,其程度与年龄及糖化血红蛋白明显相关,WI技术对其早期临床评估有重要意义。

【Abstract】 Objective To evaluate the clinical value of wave intensity technique used to early assess the elasticity of the common carotid artery in patients with type 2 diabetes mellitus (T2DM). Methods According to the course of disease, 29 patients with T2DM were divided into two groups. Group A, the course of disease was between 1 to 5 years, group B, the course of disease was longer than 5 years. Thirty healthy volunteers were included in the control group. The parameters of accelerating wave intensity(W1),decelerating wave intensity(W2),negative area(NA),R-W1,W1-W2,elasticity modulus (Eρ),stiffness index(β),arterial compliance(AC),pulse wave velocity(PWVβ)and intima media thickness (IMT) were detected by wave intensity technique. All data among three groups were recorded and compared.HbA1c,carotid artery parameters and IMT in T2DM patients were relative analyzed with WI parameters. Results Compared with control group,Eρ,β and PWVβ in group A were significantly higher (P﹤0.05),AC was significantly lower (P﹤0.01),while β,Eρ,PWVβ,IMT and W2 in group B (P﹤0.01) were all significantly higher,AC was significantly lower (P﹤0.01).There was no difference of IMT between group A and control group.Compared with group A,PWVβ and IMT in group B were all significantly higher (P﹤0.05).W1-W2 was negatively correlated to HbA1c(r= -0.56,P﹤0.01). The IMT and β were positively related with age(r=0.37,0.42,P﹤0.05). Conclusion The elasticity of the common carotid artery is decreased in T2DM patients, and the decreasing degree is correlated to HbA1c level and age.
