二维斑点追踪技术评价各年龄段正常成人左心房功能
Left atrial function evaluation of normal people in different age using 2D speckle tracking imaging
【摘要】 目的应用二维斑点追踪显像技术(2D-STI)分析各年龄段正常成人左心房应变和达峰时间,评价左心房功能,探讨其临床应用价值。方法 142名健康志愿者按年龄分为青年组、中年组和老年组,分别接受常规超声心动图检查,采集心尖四腔、心尖两腔及心尖左室长轴切面动态二维图像,应用2D-STI技术获取左心房各室壁应变曲线,测定各组左心房壁应变和达峰时间。结果各组左心房壁应变下段高于中段,中段高于上段(P﹤0.05)。随着年龄的增长,老年组心房下段、中段应变较青年组和中年组减低(P﹤0.05);而青年组与中年组比较差异无统计学意义,老年组心房上段与青年组和中年组比较差异无统计学意义。老年组心房下段达峰时间较青年组和中年组延长,老年组和中年组中段达峰时间均较青年组延长(P﹤0.05),而中年组与老年组比较差异无统计学意义,心房上段各组间比较差异无统计学意义。结论 2D-STI技术能评价各年龄段正常成人左心房功能,可为早期监测亚临床疾病、评估左心房功能提供新指标。
【Abstract】 Objective To determine the left atrial(LA) function by examining the LA strain and time-to-peak(TP) using two-dimensional speckle tracking imaging(2D-STI) and to explore the STI potential value in clinical application.Methods A total of 142 volunteers were divided into adolescence,middle-aged and elderly groups and examined by echocardiography to obtain the general data,2D apical four-chamber,apical two-chamber,and apical left ventricular(LV) longitudinal view images,followed by applying STI technique to acquire the strain curves of the LA wall to determine the strains and TPs.Results The LA strains in the three groups showed that the inferior segment was higher than the central segment and the central segment was higher than the superior segment.With the increase of age,the strain of the inferior segment and the central segment in the elderly group was lower than that in the adolescence group and middle-aged group(P﹤0.05).There was no significant difference between adolescence group and middle-aged group.There was no significant difference between the elderly group and other two groups.With the increase of age,the TP of the inferior segment in the elderly group was longer than that in the other two groups.The elderly group and the middle-aged group showed a longer central TP than that of the adolescence group,while there was no significant difference between the elderly group and middle-aged group.There was no significant difference in the TP of the superior segment among all groups.Conclusion The 2D-STI technology can evaluate LA function in healthy adults,which provides a new approach for the early monitor of LA function of sub-clinical diseases.
