【摘要】 目的以膀胱壁作为参照,探讨经直肠超声弹性成像应变率(SR)比值对诊断前列腺癌的可行性及其应用价值。方法对50例前列腺正常男性行经直肠超声弹性成像检查,在膀胱不同充盈状态下分别测量前列腺内、外腺组织与膀胱三角区膀胱壁的SR比值。收集前列腺特异抗原升高、无明确结节并且前列腺无严重凸向膀胱的患者51例,测量前列腺可疑区域与膀胱三角区膀胱壁的SR比值,绘制ROC曲线分析最佳诊断临界点,计算曲线下面积及敏感性和特异性。结果 50例前列腺正常男性排尿前后,前列腺不同部位组织与膀胱三角区膀胱壁的SR间比较差异无统计学意义。31例前列腺增生和20例非结节型前列腺癌患者膀胱三角区膀胱壁的SR比值比较差异有统计学意义(P﹤0.05)。SR比值法诊断前列腺癌的ROC曲线下面积为0.893,诊断前列腺癌的最佳临界值为3.56,敏感性为0.750,特异性为0.905。结论使用膀胱三角区膀胱壁作为参照组织对于经直肠超声弹性成像诊断前列腺癌具有一定可行性,可临床推广。更多还原

【Abstract】 Objective To discuss the possibility and value of the transrectal ultrasound elastography strain rate(SR) in diagnosing prostate carcinoma with the bladder wall as a reference.Methods Fifty healthy cases underwent trasnrectal elastography in different filling states of bladder.The SR ratios of outer and inner gland of the proatate to the bladder wall of trigone of urinary bladder were measured.Fifty-one cases of patients were collected whose prostate specific antigen increasing,no nodules and prostate no serious convex to the bladder.The SRs of lesions to the bladder wall of trigone of urinary bladder were measured.Draw ROC curve analysis the best diagnostic critical point,the area under the concentration-time curve calculation and sensitivity and specificity.Results No significant difference was found in the SR of the prostate’s different zones.The SR of prostate cancer and benign prostatic hyperplasia had sigficant difference(P < 0.05).According to the ROC curves,the area under the curve was 0.893 and the best value to diagonsis prostate cancer was 3.56 and the sensitivity and specificity were respectively 0.750 and 0.905.Conclusion In transrectal ultrasound elastography,the bladder wall of trigone of urinary bladder is a good reference to diagnosis prostate cancer and has the feasibility,but clinical promotion.更多还原
