64排128层螺旋CT对冠心病诊断价值的荟萃分析

【摘要】 目的探讨64排128层螺旋CT对诊断冠心病的价值。方法检索2000年至2012年Pubmed、Embase、Ovid、ScienceDirect、Web of Science、中国生物医学文献数据库文献,无语言限制,以64排螺旋CT、128层螺旋CT、冠状动脉造影、冠心病、64-row CT、128-slice CT、conventional coronaryangiography、CAD(coronary artery diease)为检索关键词。由2位研究人员评价每篇文献的质量并提取数据。结果共纳入稳定性好、文献质量评估较好的6项研究。研究存在异质性,总敏感度、特异度分别为0.977[95%CI(0.962-0.987)]和0.936[95%CI(0.921-0.949)],经敏感度分析后结果无明显变化。SRCO曲线下面积为:0.991 3,Q值为0.961 9。结论 64排128层螺旋CT作为一种无创的检查方法,其显示冠状动脉病变的敏感度和特异度均较高,对临床诊断冠心病具有较好的应用价值。

【Abstract】 Objective To assess the value of 64-row 128-slice spiral CT scanning in the diagnosis of coronary heart disease.Methods The authors searched the relevant academic papers in all languages collected in Pubmed,Embase,Ovid,ScienceDirect,Web of Science and Chinese Biological Medicine database from 2000 to 2012 with the of "64-row spiral CT","128-slice spiral CT","coronary angiography","coronary heart disease" and "meta-analysis".Two researchers evaluated the quality of each article and abstracted the useful data.Results Six trials with good stability and higher quality were enrolled in this study.There was heterogeneity in the trials.The total sensitivity and specificity were 0.977[95%CI(0.962-0.987)] and 0.936 [95%CI(0.921-0.949)] respectively.After sensitivity analysis,the results showed no apparent difference.The area under curve of SRCO was 0.991 3 and the Q-value was 0.961 9.Conclusion As a noninvasive screening method,64-row 128-slice spiral CT scanning is of high value in displaying coronary artery lesions with higher sensitivity and specificity,therefore,this technique is very useful in diagnosing coronary heart disease.
