【摘要】 目的探讨伴有自杀意念的偏执型精神分裂症康复期患者的认知功能状况以及自杀意念与认知损害的关系。方法使用重复性成套神经心理状态测验(RBANS)对40例伴自杀意念的偏执型精神分裂症康复期患者和40例不伴自杀意念的偏执型精神分裂症康复期患者进行认知功能检测。结果①有无自杀意念偏执型精神分裂症康复期患者两组间年龄、性别、教育程度、婚姻状态、抑郁量表评分比较差异无统计学意义;②伴自杀意念偏执型精神分裂症康复期患者患者RBANS总分(t=3.230,P=0.002)、视觉广度分值(t=5.190,P=0.000)、即刻记忆分值(t=9.255,P=0.000)、言语功能分值(t=11.840,P=0.000)均低于不伴自杀意念患者;③Logistic回归分析结果显示,视觉广度分值(OR=1.205,P<0.05)、即刻记忆分值(OR=2.237,P<0.05)、言语功能分值(OR=1.193,P<0.05)是偏执型精神分裂症康复期患者自杀意念的危险因素。结论伴自杀意念偏执型精神分裂症康复期患者存在明显认知功能缺陷,认知功能缺陷者可能存在更高的自杀意念发生率。更多还原

【Abstract】 Objective To explore the cognitive function of paranoid schizophrenia patients with suicide ideation in the recovery stage,and the relationship between suicidal ideation and cognitive impairment.Methods A total of 40 paranoid schizophrenia patients with suicide ideation in the recovery stage and 40 paranoid schizophrenia patients without suicidal ideation were evaluated by RBANS.Results ① There were no significant differences in age,gender,education level,matrimony status and the HAMD score between the two groups.②The scores of RBANS total score(t=3.230,P=0.002),visual spatiality(t=5.190,P=0.000),instanter memory(t=9.255,P=0.000)and verbal ability(t=11.840,P=0.000)were lower in patients with suicide ideation than in patients without suicide ideation.③ Logistic regression analysis showed that the lower scores of visual spatiality(OR=1.205,P<0.05),instanter memory(OR=2.237,P<0.05)and verbal ability(OR=1.193,P<0.05) were risk factors of suicide ideation in paranoid schizophrenia patients.Conclusion Cognitive deficit may exist in paranoid schizophrenia patients with suicide ideation,and patients with cognitive deficit may have higher suicide ideation.更多还原
