维持性血液透析患者血浆色氨酸及其代谢产物的分析

【摘要】 目的:探讨维持血液透析的尿毒症患者血浆色氨酸(tryptophan,TRP)代谢状态,并评价血液透析对尿毒症患者TRP代谢的纠正效果。方法:用高效液相色谱法测定28例维持血液透析的尿毒症患者透析前后和35例正常对照血浆中TRP、犬尿氨酸(kynurenine,KYN)和5-羟色胺(5-hydroxytryptamine,5-HT)的浓度,并比较尿毒症患者透析前后及其与正常对照之间的差异。结果:尿毒症患者透析前血浆中KYN、5-HT的浓度和KYN/TRP比值均较正常升高,TRP较正常降低,且差异均具有统计学意义(P<0.01),透析后KYN、5-HT的水平以及KYN/TRP比值均有明显下降,但TRP水平仍较正常对照明显降低,5-HT的浓度以及KYN/TRP比值仍较正常对照明显增高(P<0.01)。结论:尿毒症患者存在明显的TRP代谢紊乱,透析对其有一定的纠正效果。

【Abstract】 Objective:To analyze tryptophan(TRP) catabolism in uremic patients undergoing hemodialysis and to evaluate the effect of hemodialysisin uremic patients.Methods:TRP and its key metabolites kynurenine(KYN) and 5-hydroxytryptamine(5-HT) were determined by HPLC in 28 uremic patients undergoing hemodialysis and 35 healthy controls.The differences among 3 groups(control group,pre-hemodialysis group and post-hemodialysis group) and the discrepancy between each two groups were compared.Results: Levels of 5-HT,KYN and the Kyn/Trp ratio in uremic group were significantly elevated compared with control group(p<0.01),and TRP levels were significantly lower than control group(P<0.01).5-HT,KYN and the KYN/TRP ratio have decreased after hemodialysis,however,TRP,5-HT levels and the Kyn/Trp ratio remained remarkably different from controls(P<0.01).Conclusion:The metabolism of TRP was disturbed in the patients of uremia,which was greatly improved after hemodialysis in uremic patients.
