【摘要】 目的探讨微创显微手术对脑胶质瘤的治疗方案及临床疗效。方法回顾2005年~2011年来我院进行治疗的228例脑胶质瘤患者,根据患者具体情况分别进行不同程度的肿瘤切除,切下肿瘤均做术中病理切片,术后观察手术治疗的有效率。未完全切除的患者随机分为化疗组和放疗组,比较两类患者的复发率及毒副作用情况。结果 228例患者行脑胶质瘤切除术,其中Ⅰ级切除24例(10.53%),Ⅱ级切除38例(16.67%),Ⅲ级别切除69例(30.26%),Ⅳ级切除84例(36.84%),Ⅴ级切除13例(5.70%)。术后发生完全性失语4例(1.75%),偏瘫5例(2.19%),无死亡事件发生。A(化疗组)、B(放疗组)两组患者有效率分别为91.56%、95.18%,比较差异无统计学意义(P>0.05);两组复发率分别为32.26%、12.90%,比较差异具有统计学意义(P<0.05);两组患者的并发症比例分别为34.94%、4.82%,比较差异具有统计学意义(P<0.01)。结论微创显微手术辅以化疗或放射治疗对脑胶质瘤的治疗效果满意,有效率较高,但化疗与放射治疗相比毒副作用大且复发率高,因此建议临床上首选放射疗法。更多还原

【Abstract】 Objective To explore the clinical outcomes and treatment programs of minimally invasive microsurgery of glioma. Methods 2005~2011, 228 cases of glioma patients, according to the specific circumstances of the patients were tumor resection. Not completely resected portion of patients given adjuvant treatment of chemotherapy, another part of the receiving radiotherapy. Comparing the relapse rate and side effects of two groups. Results 228 patients, I grade resection is 24 cases(10.53%), the II class resection is 38 cases(16.67%), III level of resection is 69 cases(30.26%), grade IV resection is 84 cases(36.84%) and the V class resection of 13 cases(5.70%), after 4 cases(1.75%) complete aphasia, hemiplegia 5 cases(2.19%), and no deaths occurred. A(chemotherapy), B(radiotherapy) patients effective rate of 91.56% and 95.18%, the difference was not statistically significant(P>0.05); two groups recurrence rates were 32.26% and 12.90%, a statistically significant difference(P<0.05); proportion of complications in the two groups were significant differences(P<0.01). Conclusion The effect of minimally invasive micro-surgery combined with chemotherapy or radiotherapy treatment for glioma is satisfied, efficient, but the side effects and the recurrence rate of chemotherapy were high, it is recommended to promote radiotherapy adjuvant therapy.更多还原
