【摘要】 目的探讨小切口微创手术治疗跟骨关节内骨折的临床效果。方法选择2007-06～2010-06间我院收治的78例跟骨关节内骨折患者,按区间随机分配法分为小切口组(n=26),采用跟腱旁小切口克氏针内固定法;L切口组(n=26),采用L切口钢板内固定法;保守组(n=26),采用穿针撬拨复位术。比较分析3组患者出院后12～15个月的跟骨高度、宽度、长度、Bohler角及Gissane角、踝-后足功能评分等。结果 3组术后并发症发生率,随访12～15个月复查跟骨高度、宽度、长度、Bohler角及Gissane角、踝-后足功能评分,差异具有统计学意义,以小切口组改善最明显(P<0.01)。结论小切口微创治疗手术创伤小,能较好地恢复距下关节面的解剖关系,可作为SandersⅡ、Ⅲ、Ⅳ型跟骨内关节骨折治疗首选方法。更多还原

【Abstract】 Objective To explore the effect of treating intra articular fracture of calcaneus with small incision operation.Methods 78 cases of intra articular fracture patients treated in the hospital were selected from June 2007 to June 2010.According to interval randomly distribution,they were divided into small incision group(n = 26) who used small incision by achilles tendon and internally fixed with Kirschner wire were in Lincision group(n = 26) who used"L"incision and internally fixed with steel plate were in conservative group(n = 26) who used pinning prying reset operation.Calcaneal height,width,length,Bohler angle and Gissane angle,ankle foot function scores followed-up 12 ～ 15 months among 3 groups were comparatively analyzed.Results The incidence of postoperative complications,calcaneal height,width,length,Bohler angle and Gissane angle,ankle foot function scores followed-up 12 ～ 15 months among 3 groups had statistics significance differences,those in the small incision group were improved most significantly(P < 0.01).Conclusion Treated small operation by small incision has small wound.It can better restore subtalar articular surface anatomy,and can be used as the preferred method in SandersⅡ,Ⅲ,Ⅳcalcaneal intra-articular fracture.更多还原
