【摘要】 目的探讨压力带与抗栓泵对妇科盆腔手术后患者下肢深静脉血栓形成的预防作用。方法将150例妇科盆腔手术患者随机分为对照组、观察组和肝素组,每组各50例,对照组采取踝泵运动等常规下肢深静脉血栓形成干预措施;观察组在常规干预的基础上加用抗栓泵及压力带;肝素组在常规干预措施的基础上加用抗栓泵及低分子肝素。结果观察组术后凝血功能各项指标均在正常范围,肝素组术后凝血酶原时间及活化部分凝血酶时间显著延长并超出正常高值。观察组、肝素组预防下肢深静脉血栓形成效果优于对照组(P<0.01)。结论压力带与抗栓泵联合使用可增加下肢静脉回心血流量,改善血液高凝状态,可减少妇科盆腔手术患者下肢深静脉血栓的发生,并且肝素组无潜在的出血风险,值得临床推广使用。更多还原

【Abstract】 Objective To evaluate the prevention effects of graduated compression stockings( GCS) combined with intermittent pneumatic compression( IPC) on incidence of postoperative lower extremity deep venous thrombosis( LEDVT) in gynecological surgery patients.Methods A total of 150 patients undergoing gynecological surgery were recruited and randomly divided into the control group,the observation group and the heparin group,with 50 cases in each group.The control group received conventional LEDVT ankle pump exercises,the observation group used GCS and IPC besides the routine intervention,the heparin group received IPC and low molecular-weight heparin( LMWH) besides the routine intervention.Results All the postoperative coagulation observation indexes in the observation group were in the normal ranges.The prothrombin time( PT) and the activated partial thromboplastin time( APTT) in the heparin group prolonged and surpassed the normal.The prevention effects of LEDVT in the observation group and the heparin group were significantly better than that of the control group( P﹤0.01).Conclusion The GCS combined with IPC can increase the venous return of lower extremities,ameliorate the increased blood coagulability state and decrease the incidence of LEDVT in gynecological surgery patients.Besides,it has no bleeding risk of using heparin and is worthy to be recommended for clinical use.更多还原
