【摘要】 目的评价舒芬太尼-氯胺酮-异丙酚复合静脉麻醉在小儿泌尿外科手术中的应用。方法择期行小儿泌尿外科手术患儿40例,随机数字表法分为芬太尼组(F组)和舒芬太尼组(S组)。麻醉诱导时F组静脉注射芬太尼2μg/kg,S组静脉注射舒芬太尼0.2μg/kg。记录患儿入室时(T0)、麻醉诱导后5 min(T1)、手术切皮时(T2)以及术毕(T3)时的HR、MAP、SpO2。记录手术时间、术中氯胺酮追加量、苏醒时间、术后躁动及恶心呕吐、寒战、呼吸抑制等不良反应的发生情况。结果 S组在T1、T2时刻HR、MAP明显低于F组,S组术中氯胺酮追加量为(28.0±10.5)mg,明显低于F组的(39.5±14.6)mg(P<0.05)。S组躁动发生率为10%,明显低于F组的35%。结论舒芬太尼-氯胺酮-异丙酚复合静脉麻醉显示出更稳定的心血管效应、更少的术后躁动发生率,具有较好的临床应用价值。更多还原

【Abstract】 Objective To investigate the application of sufentanil-ketamine-propofol combined intravenous anesthesia in pediatric urologic surgery.Methods Forty children scheduled for urologic surgery were randomly allocated into two groups: sufentanil group(group S,n=20) and fentanyl group(group F,n=20).During anesthesia induction,0.2 μg/kg sufentanil was injected in group S,and 2 μg/kg fentanyl was injected in group F.The HR,MAP,SpO2 were recorded at admission(T0),5 min after anesthesia induction(T1),after skin incision(T2) and at the end of surgery(T3).Intraoperative rescue ketamine requirements,operation time,emergence time and the incidence of postoperative adverse events were recorded.Results Compared with group F,HR,MAP at T 1 and T2 were significantly lower in group S(P<0.05).The rescue ketamine requirements in group S was(28.0±10.5) mg,significantly lower than that in group F.The incidence of agitation in group S(10%) was significantly decreased,compared with group F(35%).Conclusion Sufentanil-ketamine-propofol combined intravenous anesthesia results in more stable hemodynamics and less postoperative agitation,which can be well used in pediatric urologic surgery.更多还原
