【摘要】 目的探讨亲属活体肾移植供肾选取的策略和围手术期处理体会。方法回顾性分析安徽省立医院2008年3月～2012年5月所完成亲属活体供肾切除术121例。供体年龄38～71岁,男43例,女78例。所有供体经过术前充分评估,选取GFR相对较低的一侧肾脏,手术采取经腹开放取肾方式,术中尽可能保留肾脏变异血管。结果 121例手术均成功完成。手术时间平均135min,无输血病例;所有供肾开放血流后血供良好,无局部缺血现象;供体平均住院时间11.2d。术后随访6～36个月,所有供体血清肌酐水平正常,全部恢复正常工作和生活。结论供体安全性是活体肾移植工作的"重中之重"。术前应认真全面评估供体的心理、生理及解剖状态,手术应选择术者最有把握的取肾方式以保证供体的安全和供肾的质量。更多还原

【Abstract】 Objective To explore the donor selective stratgies and perioperative treatment in living kidney transplantation.Methods 121 cases of relative living donor nephrectomy completed in Anhui Provincial Hospital from March 2008 to May 2012 were retrospective analysised.Among the 121 donors,there were 43 males and 78 females,aging from 38 to 71 years.After full preoperative assessment,the GFR relatively low side of the kidney in all donors were selected.The kidney was obtained by open nephrectomy,and the variant vessels of transplanted kidney were retained as much as possible during surgery.Results The operations of all living donors were successful.Mean operative time were 135 min,and no blood transfusion cases.All the blood flow of transplanted kidney were satisfied and no local ischemic appearance.The mean hospital stay was 11.2 d.After follow-up for 6-12 months,the serum creatinine levels of all donors were normal,and all returned to normal work and life.Conclusion The safety of donors is the "top priority"in living donor kidney transplantation.Before surgery,the psychological,physiological and anatomical status of donors should be careful and comprehensive assessed.During the operation,the surgeon should choose his best way to ensure the safety of donors and the quality of transplanted kidney.更多还原
