胰岛素盐水微量泵在糖尿病患者腹部手术后肠外营养期间的应用

【摘要】 目的观察胰岛素盐水微量泵在糖尿病患者腹部手术后肠外营养期间的治疗效果。方法选择腹部手术后行完全胃肠外营养(TPN)治疗1周(或以上)的糖尿病患者60例,分成两组:胰岛素盐水微量泵静脉滴注组(试验组)和传统胰岛素加入TPN液组(对照组),分别监测术后7d内输注TPN过程中血糖水平,胰岛素用量及低血糖和低血钾发生情况。结果治疗后第1~5天试验组血糖控制情况明显优于对照组;治疗后第6天和第7天两组血糖比较差异无统计学意义;试验组胰岛素用量和低血糖及低血钾发生率均低于对照组。结论胰岛素微量泵在治疗糖尿病患者腹部手术后TPN治疗期间,与对照组相比,血糖控制平稳,发生低血糖和低血钾情况少,并且每日胰岛素用量更精确,临床应用前景良好。

【Abstract】 Objective To observe the therapeutic effect of the insulin and saline input with microinfusion pump in diabetic patients after abdominal operation during parenteral nutrition. Methods 60 case of diabetic patients after abdominal operation were divided into 2 groups randomly. One group received continuous insulin and saline vein input with microinfusion pump (the test group) and the other group received a certain amount of insulin with parenteral nutritional liquid(the contrast group).All patients were treated with total parenteral nutrition(TPN) for one week or more. Plasma glucose,the dosage of insulin, hypoglycemia incidence and low serum potassium incidence in seven days after the operation were monitored. Results The plasma glucose of the test group was better than that of the contrast group at the first day to the fifth day and there was no significant difference from the sixth day to the seventh day. The dosage of insulin, hypoglycemia incidence rate and low serum potassium incidence rate of the test group were lower than those of the contrast group. Conclusion For diabetic patients after abdominal operations during the total parenteral nutrition, insulin and saline microinfusion pump is better than the contrast group in controlling plasma glucose, declining hypoglycemia incidence and low serum potassium incidence. And it makes the dosage of insulin more accurate, the prospect of this method in clinical is very well.
