【Abstract】 In order to realize the healthcare model where "people go to community healthcare facilities for minor diseases,hospital for major diseases and community healthcare facilities for rehabilitation" and in view of the limitations and shortcomings in the current two-way referral system,Ximen Subdistrict Health Service Center and the Second Hospital of Ningbo tried to unify standards,strengthen physician communication,integrate medical resources and promote health management and health education to improve the two-way referral system.更多还原
