【摘要】 目的通过动态监测危重症患者的血清半乳甘露聚糖(GM),评价其对曲霉菌感染早期诊断的临床价值。方法对391例重症监护病房收治的患者每隔2d进行一次GM动态监测,同时进行痰培养及痰涂片的真菌检查,参照不同的吸光度指数(AI),对GM检测结果进行分析,比较用药前、后GM的变化。结果当AI≥0.5,GM检测敏感性为92.68%(76/82);当AI<0.5,GM检测特异性为100.00%(309/309)。抗真菌治疗后,血清GM水平呈下降趋势。结论动态检测GM对危重症患者曲霉菌感染的早期诊断具有重要临床意义。更多还原

【Abstract】 Objective To monitor the serum galactomannan(GM) of critically ill patients dynamically and estimate its clinical value of early diagnosis of Aspergillus infections.Methods GM dynamic monitoring were performed on 391 patients in intensive care unit every 2 days,and sputum culture and smear for detection of Aspergillus were conducted simultaneously.The results of GM testing was analyzed with reference of different absorbent index(AI),and GM changes before and after medication were compared.Results Sensitivity of GM detection was 92.68%(76/82) when AI≥0.5,and its specificity was 100.00%(309/309) when AI<0.5.Serum GM level showed a declining trend after antifungal therapy.Conclusion Dynamic detection of GM has clinical significance in early diagnosis of Aspergillus infections in critically ill patients.更多还原
