饮茶型氟中毒病区居民经砖茶摄氟量与氟骨症病情相关分析

【摘要】 目的分析饮茶型氟中毒居民经砖茶摄氟量与X线氟骨症病情关系。方法选择内蒙古克什克腾旗饮茶型氟中毒病区达来诺日镇罕达罕、哈达英格嘎查为调查点,对调查点20~70岁居民进行X线氟骨症检查及砖茶饮料含氟量检测,计算经砖茶日摄氟量、相对总摄氟量[经砖茶饮料日摄氟量-3×365 d×(年龄-20)];X线氟骨症诊断采用WS192-2008《地方性氟骨症诊断标准》,砖茶饮料含氟量检测采用离子选择电极法;分析经砖茶日摄氟量、相对总摄氟量与X线氟骨症检出率关系。结果调查点居民X线氟骨症检出率为61.7%(82/133),X线重度氟骨症为3.8%(5/133),调查点为地方性氟中毒重病区。调查点砖茶饮料含氟量(2.55±1.29)mg/L;每人每天经砖茶摄氟量(6.92±5.29)mg,相对总摄氟量(64.07±59.91)g/人,经砖茶摄氟量、相对总摄氟量与X线氟骨症检出率均呈正相关(R分别为0.908、0.878,F分别为14.09、10.13,P<0.05)。每人每天经砖茶摄氟量≤3.0 mg与>3.0 mg各组X线氟骨症检出率比较无显著性差异,相对总摄氟量>15 g/人X线氟骨症检出率(57.69%)是显著升高的节点(t=5.08,P<0.05),如按40岁年龄计算,每人每天经砖茶摄氟量为2.05 mg[153÷365÷(40-20)],相对总摄氟量≤15g/人与>15.0 g/人各组X线氟骨症检出率比较均有显著性差异(χ2值分别为5.08、6.83、10.78、11.32,P<0.05)。结论经砖茶摄氟量、相对总摄氟量与X线氟骨症检出率均呈显著正相关,相对总摄氟量≥15 g/人X线氟骨症检出率显著增高,提示每人每天经砖茶摄氟量应≤2.0 mg。

【Abstract】 Objective To analyze the correlation between brick-tea intake of fluoride and radiographic skeletal fluorosis of residents in brick-tea type fluorosis region.Methods Handahan gacha of Dalainori town in Inner Mongolia keshiketongqi was selected as the investigation spot to make a radiographic skeletal fluorosis examination and brick-tea beverage fluorine content detection for the residents aged from 20 to 70,calculating their daily intake of fluoride brick tea and relative total intake of fluoride in brick tea[(daily intake of fluoride by drinking brick tea-3×365 d×(age-20)];X-ray diagnosis of skeletal fluorosis was in accordance with WS192-2008 "endemic skeletal fluorosis diagnostic criteria",and the brick-tea fluorine content was detected by ion-selective electrode method to analyze the corelation between the brick-tea intake of fluoride,relative total intake of fluorid and radiographic skeletal fluorosis.Results The detection rate of radiographic skeletal fluorosis for the investigated residents was 61.7% (82/133),X-ray severe skeletal fluorosis 3.8%(5/133),which proved the investigated spot was the severe endemic fluorosis region.The fluorine content for brick-tea beverage was(2.55 +1.29) mg/L;the brick-tea fluoride intake was(6.92 + 5.29) mg(person/day),relative total intake of fluoride was(64.07 + 59.91) g/person.The brick-tea fluoride intake,relative total intake of fluoride and X-ray fluoride bone disease detection rates were positively correlated (R=0.908,0.878,F=14.09,10.13,P<0.05).The groups of radiographic skeletal fluorosis with brick-tea fluoride intake less than or equal to 3 mg(person/day) and >3 mg(person/day) didn’t show significant difference in detection rate.The detection rate for relative total fluoride intake>15 g/person with X-ray bone fluoride syndrome (57.69%) increased significantly.Node(< = 5.08,P<0.05),calculated by age of 40,the brick-tea fluoride intake was 2.05 mg(person/day)[(153/365/(40-20)],relative total intake of fluoridel5 g/person and>15.0 g/ person,so in each group X-ray fluoride bone disease detection rate was significantly different(x2=5.08,6.83, 10.78,11.32,P<0.05).Conclusion The brick-tea daily fluoride intake,relative total intake of fluoride and radiographic skeletal fluorosis detection rates were significantly correlated;X-ray of fluorine bone disease detection rate for relative total intake of fluoride over 15.Og/person increased significantly,which showed that brick-tea fluoride intake should be less than or equal to 2 mg(person/day).
