【摘要】 目的:探讨慢性鼻咽炎的MRI表现及与鼻咽癌的鉴别诊断。方法:回顾性分析经活检或/和临床随访证实的32例慢性鼻咽炎和67例鼻咽癌的影像学特征,讨论其MRI表现的异同点。结果:32例慢性鼻咽炎中有26例MRI表现为鼻咽顶壁、后壁、双侧壁、咽鼓管圆枕粘膜弥漫性、对称性肿胀增厚,病变呈等T1稍长T2异常信号,信号均匀,表面光滑;无肌层受累,肌间隙及咽旁脂肪间隙形态、信号正常,无受压、推移征象;无颅底骨质破坏;增强扫描为病灶呈均匀显著强化,粘膜线完整。67例鼻咽癌中有61例MRI表现为鼻咽腔不对称狭窄或肿物隆突,表面不规则,粘膜层和肌层、肌间隙同时受累;病变多呈等T1稍长T2异常信号,病变信号不均匀,多有咽旁间隙受累及受压、推移改变;多有颅底骨质破坏;增强扫描为病灶呈中等不均匀或均匀强化。结论:MRI检查在慢性鼻咽炎的诊断以及慢性鼻咽炎和鼻咽癌的鉴别诊断中可以提供可靠的信息。更多还原

【Abstract】 Objective:To study the MRI manifestations and differential diagnosis value of chronic nasopharyngitis and nasopharyngeal carcinoma.Methods:MRI findings of 32 patients with chronic nasopharyngitis and 67 patients with nasopharyngeal carcinoma pathologically and/or clinically confirmed were analyzed retrospectively,the similarities and differences of MR imaging findings were discussed.Result:Of the 32 cases with chronic nasopharyngitis,26 cases nasopharynx walls and tubal torus mucosa hdiffusible and symmetry had swelling.The lesions demonstrated isointensity on T1-weighted images,and slight hyperintensity on T2-weighted images,homogeneous signal,smooth surface,muscularis was not involved,parapharyngeal space was not pressured or moved,basicranial bony tissue was not decomposed,after contrast,marked homogeneous enhancement of the lesions was seen in all 21 cases.Nasopharyngeal carcinoma was found on MR imaging in 67 cases,among them,61 cases nasopharyngeal cavity displayed asymmetrical stenosis or mass,irregular surface,mucosa and muscularis was involved simultaneously.The mass demonstrated isointensity on T1-weighted images,and slight hyperintensity on T2-weighted images,heterogeneous signal.parapharyngeal space was pressured or moved mostly,basicranial bony tissue was decomposed mostly.After contrast,heterogeneous or homogeneous medium enhancement of the mass was seen in all 31 cases.Conclusion:MRI can provide reliable information for MRI finding of chronic nasopharyngitis and differential diagnosis of chronic nasopharyngitis and nasopharyngeal carcinoma.更多还原
