益气扶阳法对严重脓毒症免疫调理及预后的影响

【摘要】 目的观察具有益气扶阳作用的参芪扶正注射液对严重脓毒症气虚证的临床疗效并探讨其作用机制。方法 60例严重脓毒症气虚证患者随机分成两组,治疗组给予参芪扶正注射液联合"拯救脓毒症战役"(SSC)经典治疗,对照组给予SSC经典治疗,分别观察两组治疗前后免疫、炎症指标变化,对比治疗前后APACHEⅡ评分及死亡率。结果 1.第1和7天同一时间点治疗组T淋巴细胞亚群中的CD4+、CD4+/C8+比值和免疫球蛋白IgG、IgA水平明显升高,CD8+T细胞和IgM水平、血清白细胞(WBC)、C反应蛋白(CRP)和降钙素原(PCT)水平显著降低(P<0.05)。2.7天后APACHEⅡ评分治疗组比对照组明显降低(P<0.05);近期死亡率两组比较治疗组低于对照组,但无显著性差异。结论益气扶阳法在严重脓毒症免疫抑制阶段能显著提高患者细胞免疫和体液免疫功能,减轻炎症反应,值得临床推广应用。

【Abstract】 Objective To observe the clinical effects of Shenqi Fuzheng injection on Qi deficiency patients with severe sepsis and explore its mechanism. Methods 60 Qi deficiency patients with severe sepsis were randomly divided into treatment group( 30 cases) and control group ( 30 cases) . Classic modern medical therapy was given to both groups. At the same time,the treatment group was intravenously injected Shenqi Fuzheng injection. The changes of laboratory parameters,APACHEⅡscores and mortality rate were observed in both groups. Results 1. T-lymphocyte subpopulations CD4+,CD4+/C8+ ratio and immuno-globulins IgG,IgA of treatment group were better improved than control group. The level of CD8+ T-lymphocyte,IgM,WBC,CRP and PCT were decreased evidently on the 7th day ( P < 0. 05) . 2. Compared with control group,the APACHEⅡscores of treatment group was decreased significantly ( P < 0. 05) . The treatment group mortality rate was less than control group,but no significant difference between the two groups. Conclusion The study indicates that Shenqi Fuzheng injection could significantly improve the immunity of patients and decrease APACHE Ⅱ scores and mortality rate. It was deserved application and recommendation for the clinic.
