适用于临床及动物试验的高频重复经颅磁刺激系统设计及其应用
Design and Application of High Frequency Repetitive Transcranial Magnetic Stimulation System for Clinical and Animal Tests
【摘要】 为开展电磁刺激技术在神经调控及神经修复中的应用研究,研制了一种适用于临床及动物试验研究的高频重复经颅磁刺激(rTMS)系统,并利用该系统研究了rTMS对神经递质的影响及对脊髓损伤修复的作用。高频rTMS系统的参数由相关理论分析和仿真计算确定。系统完成后,使用慢性rTMS对SD大鼠进行15d刺激,考察其对大鼠谷氨酸和γ-氨基丁酸能系统的作用。最后,将rTMS系统与踏车训练机同步,研究rTMS对临床脊髓损伤患者康复治疗的作用。测试结果表明:研制的rTMS系统频率为0~15Hz可调,八字(figure-eight)线圈最高磁感应强度为2.5T。动物试验结果表明:慢性rTMS能选择性地调节不同脑区谷氨酸和γ-氨基丁酸含量。临床试验结果表明,rTMS同步踏车训练治疗6周后,训练组患者的运动评分(ASIA)和脊髓损伤步行指数Ⅱ(WISCIⅡ)相比对照组高,两者具有显著性差异。由上述试验结果可知:1)研制的rTMS系统的刺激频率和磁感应强度满足设计要求;2)慢性低频经颅磁刺激可以在某些神经精神疾病类疾病治疗中发挥作用;3)不完全性胸腰段脊髓损伤患者通过经颅磁刺激同步踏车训练后,运动功能恢复可得到一定程度的促进。
【Abstract】 For studying applications of the electromagnetic stimulation technology in neuroregeneration and neural modulation,we designed a high frequency repetitive transcranial magnetic stimulation(rTMS) system which was fit for clinical trial and animal experiments to study the influence of rTMS on neurotransmitter and recoveries of spinal cord injury.Based on theoretical calculations and simulations,parameters of the high frequency rTMS system was determined.After the system was manufactured,chronic rTMS was used to stimulate Sprague-Dawley(SD) rats for 15 d so as to investigate the possible effect of rTMS on amino acid neurotransmitter of the rats.Furthermore,rTMS synchronizing treadmill training was used to study the effect of rTMS on neurofunction recovery of patients with incomplete thoracolumbar spinal cord injuries.According to the tests,the developed rTMS system has a variable frequency range of 0-15 Hz,and the maximum magnetic flux density produced by 8-type coils is 2.5 T.Results of the animal experiments show that the content of glutamate and gamma-aminobutyric acid can be selectively modulated in different brain areas.Results of the clinical trial show that,after 6 weeks of rTMS,the ASIA motor score and WISCI Ⅱ are higher in rTMS group those in control group.It is concluded that 1) the frequency and magnetic flux density of the developed rTMS system satisfy the design requirements;2) chronic rTMS can play an important role in therapies of some neurologic and psychiatric diseases;3) the motor function of patients with incomplete thoracolumbar spinal cord injury can be improved to some extent after the rTMS synchronizing treadmill training.
