18F-FDG PET-CT在非小细胞肺癌术前分期中的应用价值

【摘要】 目的:评价18F-FDG PET-CT在非小细胞肺癌患者术前分期中的临床应用价值。方法:回顾性分析经病理证实的112例非小细胞肺癌患者的18F-FDG PET-CT资料,并与病理结果及随访结果进行对照分析。结果:非小细胞肺癌原发灶主要表现为病灶部分或全部呈放射性浓聚。术前对非小细胞肺癌PET-CT T分期与术后T分期相比符合率为T1、T2、T3、T4期分别为87.50%、95.56%、90.48%、100%,T分期的总体符合率为93.75%,与最终诊断结果高度一致(kap-pa=0.929,P=0.000)。N分期的符合率N0和N3期均为100%,N1期63.16%,N2期90.91%,N分期总体符合率为91.96%,与最终诊断结果高度一致(kappa=0.928,P=0.000)。M分期诊断符合率为96.97%(kappa=0.957,P=0.000)。结论:18F-FDG PET-CT对非小细胞肺癌患者的术前TNM分期与术后分期具有高度的一致性,具有重要的临床应用价值。

【Abstract】 Objective:To study the value of 18F-FDG PET-CT for the evaluation of pre-operative clinical staging of non-small cell lung cancer(NSCLC).Methods:18F-FDG PET-CT materials of 112 patients with pathology proved NSCLC were retrospectively analyzed and correlated with the results of pathology and follow-up.Results:The main PET-CT manifestations of NSCLC were intense uptake of 18F-FDG in part or total region of tumor.Compared with the post-surgical staging,the corresponding rate of pre-surgical T staging with PET-CT for T1、T2、T3 and T4 was 87.50%,95.56%,90.48% and 100% respectively.The overall accuracy rate of T staging was 93.75%,which was highly corresponded with the final diagnosis(kappa=0.929,P=0.000).The accuracy of N staging for N0,N3 was 100% respectively,for N1 and N2 was 63.16% and 90.91% respectively.The overall accuracy of N staging was 91.96%,which was highly corresponded with the final diagnosis(kappa=0.928,P=0.000).The corresponding rate of M staging was 96.97%(kappa=0.957,P=0.000).Conclusion:The preoperative TNM staging of non-small cell lung cancer with 18F-FDG PET-CT was highly corresponded with the final diagnosis and have a significant value in clinical application.
