【摘要】 目的规范预防跌倒的流程指引,降低医疗护理的风险,减少老年患者跌倒的发生率。方法应用医疗失效模式与效应分析(HFMEA),分析老年患者跌倒的主要原因,找到潜在失效模式,对流程进行分析探讨,计算危急值,制定改进措施。结果 6个失效模式实施HFMEA管理后,危急值均较实施前显著降低。老年患者防跌倒的流程更为规范,各种风险因子与对照组相比差异有显著意义(χ2=21.573,P<0.05)。结论运用相关理论前瞻性地对老年患者跌倒全过程进行系统分析,制定改进计划并及时落实,能有效避免护理差错的发生,降低护理投诉与纠纷,提高护理质量。更多还原

【Abstract】 Objective To standardize the process guidance prevention medical care,reduce the risk,and the incidence of falls in elderly patients. Method We used the method of healthcare failure mode and effect analysis(HFMEA),for analysis main causes of falls in older,to find the potential failure mode and analysis the process,this calculation of the critical value,formulate measures for improvement. Result All six failure modes of HFMEA had be managed after implementing critical value were significantly decreased.Elderly patients to prevent tumble.flow is more normative,various risk factors compared with the control group and the differences were statistically significant(χ~2=21.573,P< 0.05). Conclusion The usesing of relevant theories for prospectively on falls in elderly,inpatients whole process to undertake systems analysis,improvement plan and to implement,we suggest that effectively prevent nursing errors,reduce nursing complaints and disputes, improve the quality of nursing care.更多还原
