【摘要】 目的观察右美托咪啶对老年胃肠患者手术后认知功能的影响。方法选取2011年6月至2013年4月我院收治的全麻下择期胃肠手术老年患者50例,随机分为对照组和观察组,每组各25例,观察组麻醉诱导前15 min静脉泵注右美托咪啶溶液,负荷剂量1μg/kg,10min泵注完,随后以0.1μg/(kg.h)维持至手术结束停止泵注。对照组采用生理盐水代替右美托咪啶以同样的方式及速度泵注。采取积极有效护理措施,运用简易精神状态检查量表(Minimum Mental State Examination,MMSE)分别于术前1d及术后1d、7d评估患者认知功能状态,计算手术后认知功能障碍(Postoperative Cognitive Dysfunction,POCD)发生率。结果两组间比较,对照组术后1 d的MMSE的评分结果与观察组比较具有统计学意义(P<0.05),术后观察组认知功能障碍发生率较对照组明显降低(P<0.05)。结论右美托咪啶围术期给药有良好的镇静作用,术间易被唤醒,且无呼吸抑制作用,对老年患者术后认知功能障碍的发生有一定防治作用。更多还原

【Abstract】 Objective To investigate the effects of Dexmedetomidine on cognitive function in elderly patients after gastrointestinal surgery.Methods The 50 cases of elderly patients undergoing elective gastrointestinal surgery under the general anestihesia.in June 2011 to April 2011 in our hospital were randomly divided into control group and observaton group,each having 25 cases.15 min before anesthesia induction,observation group was given vein pump Dexmedetomidine solution with loading dose of 0.1 μg/kg within10 min.Control group was given normal saline in the same way and speed.After taking positive and effective nursing measures,simple mental state examination(MMSE) was used to assess the cognitive function of patients in 1 d before and 1 d,7 d after operation,and calculate the incidence of cognitive dysfunction(POCD) after operation.Results Compared with that of the observation group,the MMSE score results of the control group after 1 d has statistical significance(P <0.05).The incidence of cognitive dysfunction in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Being applied during preoperative,Dexmedetomidine can be effective in sedation,easy awakening during operation without respiratory inhibition,and the prevention and cure of cognitive dysfunction in elderly patients after operation.更多还原
