含缓释淀粉的肠内营养在亚低温治疗颅脑损伤病人中的应用

【摘要】 目的:探讨含缓释淀粉的肠内营养(EN)液瑞代在亚低温治疗的颅脑损伤病人中的应用。方法:将40例亚低温治疗的颅脑损伤病人分为治疗组与对照组,每组20例。治疗组病人在常规治疗的基础上给予鼻饲瑞代,第1天500 ml,第2天1 000 ml,第3天以后1 500 ml/d。对照组病人在常规治疗的基础上给予鼻饲能全力,第1天500 ml,第2天1 000 ml,第3天以后1 500 ml/d。分析两组病人亚低温治疗期间(3 d)血糖>7.8 mmol/L的情况,以及使用胰岛素总量的差别。结果:亚低温治疗期间,治疗组病人血糖高于7.8 mmol/L的频次和胰岛素使用总量均明显低于对照组(P<0.05)。结论:含缓释淀粉的EN应用于亚低温治疗的颅脑损伤病人,可较好地控制血糖波动,减少胰岛素的总体用量。

【Abstract】 Objective: To investigate the effect of enteral nutrition containing slow-release starch(fresrubin diabetes) in traumatic brain injury patients managed by mild hypothermia.Methods: 40 traumatic brain injury patients managed by mild hypothermia,were randomly divided into the treatment group and the control group,20 patients each.Treatment group was routinely given with fresrubin by nasogastric tube.Control group was routinely given Nutrison by nasogastric route.The patients of two groups were analyzed by the times of serum glucose concentration above 7.8 mmol /L and the amount of insulin used for control of blood glucose for a period(3 days).Results: During the hypothermia period,the frequency of serum glucose concentration above 7.8 mmol /L in treatment group was significantly lower than in the control group(P<0.05),and the total insulin dose of treatment group was significantly lower than in the control group(P<0.05).Conclusion: The enteral nutrition containing slow-release starch in the treatment of traumatic brain injury patients managed by mild hypothermia can better control serum glucose fluctuations and reduce overall insulin dosage.
