低分子肝素预防老年髋关节周围骨折围术期下肢深静脉血栓的研究

【摘要】 目的观察老年髋关节周围骨折围术期使用低分子肝素预防下肢深静脉血栓的效果。方法将2007年7月至2011年7月的105例老年髋关节周围骨折的患者分为低分子肝素组(65例)和常规治疗组(40例),常规治疗组和低分子肝素组都行物理疗法预防下肢静脉血栓,低分子肝素组加用低分子肝素预防。比较两组患者下肢静脉血栓形成的情况和凝血相关指标的状态。结果低分子肝素组发生血栓事件的患者有4例(6.15%)。常规治疗组形成血栓的患者有13例(32.5%)。两组比较,差异有统计学意义(P<0.05)。术后1、10d的血红蛋白改善明显,差异有统计学意义(P<0.05)。结论对老年髋关节周围骨折的患者采用低分子肝素预防下肢深静脉血栓形成疗效十分理想。

【Abstract】 Objective To observe the effect of perioperative use of low molecular heparin(LMH)for preventing lower limb deep vein thrombosis(DVT)in elderly hip peripheral fracture.Methods 105cases of elderly hip peripheral fracture from July 2007to July 2011were divided into the LMH group(65cases)and the conventional treatment group(40cases).The two groups were performed the physical therapy for preventing lower limb DVT.The LMH group was added with LMH for preventing DVT.The statuses of DVT occurrence and the coagulation related indicators were compared between the two groups.Results In the LMH group,the thrombus events happened in 4cases,accounting for 6.15%.In the conventional treatment group,thrombosis occurred in13cases,accounting for 32.5%.The difference between the two groups had statistical significance(P<0.05).Hemoglobin on post-operative 1,10dwas obviously improved,the difference had statistical significance(P<0.05).Conclusion Using LMH has relatively ideal effect for preventing lower limb DVT in elderly hip peripheral fracture.
